摘要
目的:探讨细胞角蛋白19片段(cytokeratin-19-fragment,CYFRA21-1)的检测在慢性肾脏病(chronic kidney disease,CKD)诊断中的应用价值。方法:将2019年1—12月在南通大学附属医院行CYFRA21-1血清水平检测的患者13133例纳入研究组,另外取同年来院体检的健康受试者212例为对照组,检测两组血清CYFRA21-1水平,以2.08 ng/mL为诊断分界点,分析各疾病中CYFRA21-1水平异常率;将对照组CYFRA21-1血清浓度水平分别与CKD及其各期比较;应用受试者工作特征(receiver operating characteristic,ROC)曲线分析CYFRA21-1对CKD及其各期诊断曲线下面积(area under the curve,AUC)、敏感性和特异性。结果:CKD、胰腺癌、肺癌居CYFRA21-1检测结果异常率较高的前3位;CKD各期患者血清CYFRA21-1浓度水平均高于健康体检者(P<0.05);CKD 1期患者血清CYFRA21-1浓度与其他分期相比,差异有统计学意义(P<0.05),CYFRA21-1在CKD及CKD 1~5期中诊断的AUC值分别为0.895±0.024、0.787±0.033、0.886±0.019、0.914±0.017、0.923±0.021、0.958±0.007,敏感性分别为82.19%、85.37%、88.89%、87.30%、91.40%、96.87%,特异性分别为82.63%、64.05%、75.83%、75.08%、82.21%、84.29%。结论:CYFRA21-1水平的监测对CKD患者具有一定的临床价值,可为临床诊疗过程提供参考。
Objective:To explore the application value analysis of the cytokeratin-19-fragment(CYFRA21-1)test in chronic kidney disease(CKD).Methods:Take the 13133 patients who underwent CYFRA21-1 serum level detection in Affiliated Hospital of Nantong University from January to December 2019 as the study group and take the 212 healthy volunteers who came to our Hospital for physical examination in the same year as the control group.Then detect the serum CYFRA21-1 concentration level of the two groups.Taking 2.08 ng/mL as the diagnostic dividing point,analyze the abnormal rate of CYFRA21-1 level in each disease.Compare the serum concentration of CYFRA21-1 in the control group with CKD and its stages.Use the receiver operating characteristic(ROC)curve to analyze the sensitivity and specificity of CYFRA21-1 for CKD and its area under the curve(AUC).Results:CKD,pancreatic cancer and lung cancer are the top three abnormal rates of CYFRA21-1 detection.The serum concentration of CYFRA21-1 in all stages of CKD is higher than that in healthy volunteers,and the differences are statistically significant(P<0.05).The differences in serum concentration of CYFRA21-1 between CKD stage 1 and other stages are also statistically significant(P<0.05).The AUC values of CYFRA21-1 in CKD and each stage were 0.895±0.024,0.787±0.033,0.886±0.019,0.914±0.017,0.923±0.021 and 0.958±0.007.The sensitivities were 82.19%,85.37%,88.89%,87.30%,91.40% and 96.87%.The specificities were 82.63%,64.05%,75.83%,75.08%,82.21%and 84.29%.Conculusion:The monitor of the CYFRA21-1 has certain clinical value for CKD patients and can provide reference for clinical diagnosis and treatment.
