摘要
目的探讨不同方法治疗儿童股骨远端骨骺骨折的疗效。方法11例儿童股骨远端骨骺骨折患儿中,2例Ⅰ型均行闭合复位克氏针固定;3例Ⅱ型中2例行闭合复位克氏针固定,1例行闭合复位空心钉固定;1例Ⅲ型行闭合复位空心钉固定;5例Ⅳ型中2例行闭合复位空心钉固定,1例行闭合复位克氏针固定,2例行切开复位空心钉结合克氏针固定。根据Thomson标准评价功能。结果患儿均获得随访,时间12~37(22.8±8.1)个月。3例骨骺生长正常,8例出现骨骺生长障碍。末次随访根据Thomson标准评价功能:优6例,良3例,差2例,优良率9/11。结论根据儿童股骨远端骨骺骨折类型的不同,行闭合或切开复位,采用克氏针、空心钉或两者相结合进行固定,手术损伤较小,术后膝关节功能恢复满意。
Objective To investigate the clinical effect of distal femoral epiphyseal fractures treated by different methods.Methods The 11 cases with distal femoral epiphyseal fractures of children were selected,they were used Salter-Harris classification.Except foR2 cases treated with cannulated screw combined with KirschneRwire fixation afteRopen reduction,the otheRchildren were used the closed reduction and various internal fixation,of which 2 cases of typeⅠfractures were treated by KirschneRwires fixation;foR3 cases of typeⅡ,2 cases were treated with KirschneRwires fixation,and 1 case was performed with cannulated screws;1 case of typeⅢwas treated by cannulated screw;in 5 cases of typeⅣ,there were 2 cases of cannulated screws fixation,1 case of KirschneRwire fixation.The results were evaluated according to Thomson functional criterion.Results All 11 cases of distal femoral epiphyseal fractures were followed up foR12~37(22.8±8.1)months.The growth of epiphysis was normal in 3 cases and growth disordeRin 8 cases.At the last follow-up,the results of Thomson function evaluation were excellent in 6 cases,good in 3 cases,and pooR2 cases,with the excellent-good rate of 9/11.Conclusions According to different fracture types of distal femoral epiphysis in children,closed oRopen reduction is performed,and fixation is performed with KirschneRwire,cannulated screw,oRa combination of the two kinds of fixation,with less surgical injury and satisfactory postoperative functional recovery of the knee joint.
