摘要
目的探讨布托啡诺联合右美托咪定用于剖宫产手术术中寒颤治疗的效果。方法选取腰-硬联合麻醉后出现寒颤的行剖宫产手术的产妇70例,采用随机数表法分为两组,每组35例。观察组:35例产妇在胎儿娩出后立即给予布托啡诺(0.02 mg/kg静脉缓慢推注)以及右美托咪定(0.5μg/kg微泵静脉泵注,时间为30 min)。对照组:35例产妇在胎儿娩出后给予静脉缓慢推注0.015 mg/kg芬太尼,然后微泵静脉泵注0.5μg/kg右美托咪定,持续时间为30 min。观察两组用药后寒颤的改善情况;记录两组用药后的镇静程度;记录两组低血压、头晕、恶心、心动过缓、呼吸抑制等不良反应发生率。结果观察组寒颤治疗的有效率高于对照组,差异具有统计学意义(P<0.05);两组Ramsay镇静评分比较,差异有统计学意义(P<0.05),两组不良反应的发生情况,差异有统计学意义(P<0.05)。结论布托啡诺联合右美托咪定可有效治疗剖宫产产妇术中寒颤,镇静效果好且不良反应的发生率低。
Objective To explore the effect of butorphanol combined with dexmetomidine in the treatment of shivering during cesarean section. Methods 70 patients who received cesarean section with shivering after combined spinal-epidural anesthesia were randomly divided into two groups with 35 cases in each group(n = 35). Observation group: 35 parturients were given butorphanol(0.02 mg/kg intravenous slow injection) and right demetotomidine(0.5μg/kg micropump intravenous pump, time was 30 min) after fetal delivery. Control group: 35 parturients were given slow intravenous injection of 0.015 mg/kg fentanyl after fetal delivery, and then 0.5μg/kg right metotomidine was injected intravenously with micropump for the duration of 30 min. The relief of shivering in the two groups was observed, the degree of sedation in the two groups was recorded, and the incidence of adverse reactions such as hypotension, dizziness, nausea, bradycardia and respiratory depression in the two groups were recorded. Results The effective rate of shivering treatment in observation group was significantly higher than that in control group, and there was significant difference in Ramsay sedation score and adverse reactions between the two groups. Conclusion Butorphanol combined with dexmedetomidine is effective in the treatment of shivering during cesarean section, with good sedative effect and low incidence of adverse reactions.
