摘要
目的观察负压机械刺激对脑卒中后肩手综合征Ⅰ期的改善状况。方法选取2019年3月至2020年9月在中山市人民医院神经内科、康复医学科、中医科住院,病程1~3个月,且临床症状符合肩手综合征Ⅰ期的患者60例为研究对象,随机分为治疗组(n=30)和对照组(n=30),两组均进行常规的临床药物治疗以及康复锻炼,治疗组增加负压机械刺激治疗。比较治疗2周及4周后患手的肿胀值、VAS评分、上肢FMA评分、MBI评分。结果治疗2周及4周后,治疗组的VAS评分、患手的肿胀值均明显低于对照组,差异有统计学意义(P<0.05);此外,治疗组的FMA评分及MBI评分均高于对照组,差异有统计学意义(P<0.05)。结论负压机械刺激能更进一步减轻肩手综合征Ⅰ期患者的水肿及疼痛,对提高上肢运动功能及改善日常生活质量有显著疗效。
Objective To observe the improvement of stageⅠpost-stroke shoulder-hand syndrome with negative pressure mechanical stimulation.Methods A total of 60 patients hospitalized in the Department of Neurology,Department of Rehabilitation Medicine and Department of Traditional Chinese Medicine of Zhongshan City People’s Hospital from March 2019 to September 2020 with a disease course of 1-3 months whose clinical symptoms were consistent with stageⅠshoulder-hand syndrome were randomly divided into the treatment group(n=30)and the control group(n=30).Both groups received routine clinical drug treatment and rehabilitation exercise and the treatment group was supplemented with negative pressure mechanical stimulation.The swelling value,Visual Analogue Scale(VAS)pain score,Fugl-Meyer Assessment(FMA)score for upper extremity and Modified Barthel Index(MBI)score of the affected hand after 2 weeks and 4 weeks of treatment were compared.Results After 2 weeks and 4 weeks of treatment,the VAS score and swelling value of the affected hand in the treatment group were significantly lower than those in the control group,with statistically significant differences(P<0.05).In addition,the FMA score and MBI score of the treatment group were higher than those of the control group,with statistically significant differences(P<0.05).Conclusion Negative pressure mechanical stimulation can further relieve the edema and pain of patients with stage I shoulder-hand syndrome,and has a significant effect on improving the motor function of the upper extremity and promoting the quality of daily life.
