摘要
目的分析磁共振成像(MRI)在直肠癌术前浸润深度及范围(T分期)与淋巴结转移(N分期)中的临床诊断价值。方法选择2019年2月至2021年2月于廉江市人民医院治疗的84例直肠癌患者。患者均行MRI检查。观察MRI诊断TN分期结果,并以病理结果作为金标准,计算MRI诊断TN分期准确率及与病理结果的一致性,比较不同T分期患者的表观扩散系数(ADC)值、回流速率常数(Kep)与容积分数(Ve)。结果MRI诊断T分期准确率为80.95%,与病理诊断结果一致性较为理想(Kappa=0.696);MRI诊断N分期准确率为55.95%,与病理诊断结果一致性差(Kappa=0.320);T4期与T3期患者的ADC值低于T1+2期患者,T4期与T3期患者的Kep、Ve大于T1+2期患者,差异有统计学意义(P<0.05)。结论MRI在直肠癌术前N分期诊断准确率较低,在诊断T分期准确率较高,能够通过测定ADC值、Kep与Ve对直肠癌T分期进行诊断,为直肠癌分期诊断及治疗提供依据。
Objective To analyze the clinical diagnostic value of magnetic resonance imaging(MRI)in preoperative depth and extent of infiltration in rectal cancer(T staging)and lymph node metastasis(N staging).Methods A total of 84 patients with rectal cancer treated at Lianjiang People’s Hospital from February 2019 to February 2021 were selected as study subjects,and all underwent MRI examination.The TN staging results diagnosed by MRI were observed,and the accuracy of MRI in diagnosing T staging and the consistency between the TN staging results with the pathological findings was calculated by taking the pathological findings as the gold standard.The apparent diffusion coefficient(ADC)values,reflux rate constant(Kep)and volume fraction(Ve)were compared among patients with different T staging.Results The accuracy of MRI in diagnosing T staging was 80.95%,with good consistency with pathological findings(Kappa=0.696).The accuracy of MRI in diagnosing N staging was 55.95%,with poor consistency with pathological findings(Kappa=0.320).The ADC values were smaller and the Kep and Ve were greater in patients of T4 and T3 stages than those in patients of T1 and T2 stages,with statistically significant differences(P<0.05).Conclusion MRI has a lower accuracy rate in diagnosing preoperative N staging and a higher accuracy rate in diagnosing T staging of rectal cancer,and can diagnose T staging of rectal cancer by measuring ADC value,Kep and Ve,which provides a basis for staging diagnosis and treatment of rectal cancer.
