摘要
目的:探讨使用经皮脊柱内镜腰椎间盘摘除术(percutaneous endoscopic lumbar discectomy,PELD)对老年腰椎间盘突出症(lumbar disc herniation,LDH)患者满意度及优良率的影响。方法:选取2019年3月-2021年3月广州市番禺区中心医院收治的86例LDH患者,按照随机数字表法将患者分为研究组和对照组,每组43例。对照组接受腰椎间盘摘除植骨融合内固定术,研究组接受PELD术。比较两组手术相关指标、视觉疼痛模拟(VAS)评分、优良率、满意度。结果:研究组切口长度、手术时长及住院时长均短于对照组,术中出血量少于对照组,差异均有统计学意义(P<0.05)。术前、术后3个月,两组VAS评分比较,差异均无统计学意义(P>0.05)。术后3 d,研究组VAS评分低于对照组,差异有统计学意义(P<0.05)。术后3个月,两组优良率、满意度比较,差异均无统计学意义(P>0.05)。结论:通过实施PELD术可给予LDH患者有效治疗,可获得与传统开放手术相当的治疗效果,但是PELD术的出血量更低,且术后短期疼痛感更小。
Objective:To investigate the effect of percutaneous endoscopic lumbar discectomy (PELD) on satisfaction and excellent rate of elderly patients with lumbar disc herniation (LDH).Method:A total of 86 LDH patients admitted to Guangzhou Panyu Central Hospital from March 2019 to March 2021 were selected,and they were divided into study group and control group according to random number table method,with 43 patients in each group.The control group received lumbar intervertebral disc excision and bone graft fusion and internal fixation,while the study group received PELD.Operation related indexes,visual pain analog scale (VAS) score,excellent rate and satisfaction were compared between the two groups.Result:The length of incision,length of operation and length of hospital stay in the study group were shorter than those in the control group,and the amount of intraoperative blood loss in the study group was less than that in the control group,the differences were statistically significant (P<0.05).There were no significant differences in VAS scores between two groups before surgery and 3 months after surgery (P>0.05).3 d after surgery,VAS score of the study group was lower than that of the control group,the difference was statistically significant (P<0.05).3 months after surgery,there were no significant differences in the excellent rate and satisfaction between two groups (P>0.05).Conclusion:PELD can give effective treatment to patients with LDH,and can obtain the same therapeutic effect as traditional open surgery,but PELD has lower bleeding and less postoperative short-term pain.
