摘要
目的 比较左炔诺孕酮宫内缓释系统(levonorgestrel-releasing intrauterine system,LNG-IUS)和手术两种方式治疗剖宫产术后子宫瘢痕憩室(cesarean scar diverticulum,CSD)患者的疗效及并发症情况。方法 回顾性分析2015年11月至2021年2月在武汉市武昌医院接受治疗的56例有症状CSD患者的临床治疗资料,其中LNG-IUS治疗组26例,手术治疗组30例。收集一般资料(年龄、剖宫产次数、主要临床症状),CSD评分,比较两组间治疗后症状改善情况、相关并发症、治疗费用等。结果 两组患者的年龄、剖宫产次数,主要临床症状、CSD评分、临床症状改善效果比较差异无统计学意义(P> 0. 05)。但手术治疗组相关并发症发生率及平均治疗费用明显高于LNG-IUS治疗组(P <0. 05),治疗6月后每月阴道出血时间LNG-IUS治疗组短于手术治疗组(P <0. 05)。结论 长期应用LNG-IUS治疗和手术治疗均能有效缓解患者的临床症状,疗效相当,但LNG-IUS治疗更为简单和经济,可作为无生育需求的有症状CSD患者的主要治疗方法。
Objective To compare the efficacy and complications of levonorgestrel-releasing intrauterine system(LNG-IUS)and surgery in the treatment of cesarean scar diverticulum(CSD)patients.Methods The clinical treatment data of 56 patients with symptomatic CSD who were treated in Wuhan Wuchang Hospital from November 2015 to February 2021 were retrospectively analyzed.Among them,26 patients were in the LNG-IUS treatment group,and 30 patients were in the surgery group.General information(age,frequency of cesarean section,main clinical symptoms),CSD score,symptom improvement after treatment,related complications and treatment costs were compared between the two groups.Results There were no significant differences in age,cesarean section times,main clinical symptoms,CSD score and clinical symptom improvement effect between the two groups(P>0.05).However,the related complications and average treatment costs in the surgery group were significantly higher than those in the LNG-IUS treatment group(P<0.05).After 6 months of treatment,the monthly vaginal bleeding time in the LNG-IUS treatment group was shorter than the surgery group(P<0.05).Conclusion Long-term application of LNG-IUS therapy and surgical treatment can effectively relieve the clinical symptoms of patients,and the curative effects are equivalent,but LNG-IUS therapy is simpler and more economical,and can be used as a primary method for patients with symptomatic CSD and without fertility requirements.
