摘要
目的分析超声骨刀配合高速涡轮机微创术对下颌阻生牙患者视觉模拟评分法(VAS)评分与拔牙窝完整性的影响。方法选取2020年1—12月该院收治的下颌阻生牙患者100例,随机摸球法分为两组,对照组50例为传统拔牙术,观察组50例为超声骨刀配合高速涡轮机微创术。对比两种不同治疗方式下VAS评分与拔牙窝完整性。结果较对照组,术后1、2、3个月观察组患者拔牙创牙槽骨吸收量(0.85±0.13)、(0.52±0.12)、(0.30±0.10)mm均更低,差异有统计学意义(t=12.872、20.708、16.474,P<0.05);治疗1、2 d后,观察组患者疼痛评分(5.21±0.8)分、(3.21±1.10)分显著低于对照组,差异有统计学意义(t=5.379、7.597,P<0.05);观察组患者拔牙窝完整性以及开口受限程度、肿胀度(1.52±0.34)、(3.70±0.40)mm、(14.76±3.15)mm均低于对照组,差异有统计学意义(t=31.596、21.282、4.549,P<0.05);观察组患者术后并发症发生率4.00%低于对照组的18.00%,差异有统计学意义(χ^(2)=5.005,P<0.05)。结论为下颌阻生牙患者采用超声骨刀配合高速涡轮机微创术进行治疗时不仅可有效降低患者疼痛评分,同时也可保持拔牙窝完整性,减少骨吸收,降低术后并发症。
Objective To analyze the effect of ultrasonic bone knife combined with high-speed turbine minimally invasive surgery on the VAS score and the integrity of the extraction socket in patients with mandibular impacted teeth.Methods A total of 100 patients with impacted mandibular teeth admitted to the hospital from January to December 2020 were selected.Random ball touch was divided into two groups,50 cases in the control group were traditional tooth extractions,and 50 cases in the observation group were ultrasonic bone knife combined with high-speed turbine minimally invasive surgery.Compared the VAS score and the integrity of the extraction socket under two different treatments.Results Compared with the control group,the alveolar bone resorption(0.85±0.13)mm,(0.52±0.12)mm,(0.30±0.10)mm of the patients in the observation group 1,2,3 months after suryery was lower,the difference was statistically significant(t=12.872,20.708,16.474,P<0.05).The pain scores(5.21±0.8)points,(3.21±1.10)points of the observation group 1 d and 2 d after treatment were significantly lower than those of the control group,and the difference was statistically significant(t=5.379,7.597,P<0.05).The integrity of the extraction socket,the degree of mouth opening restriction,and the degree of swelling(1.52±0.34),(3.70±0.40)mm,(14.76±3.15)mm of the observation group were lower than those of the control group,the difference was statistically significant(t=31.596,21.282,4.549,P<0.05).The incidence of postoperative complications in the observation group was 4.00%lower than that in the control group 18.00%,and the difference was statistically significant(χ^(2)=5.005,P<0.05).Conclusion The use of ultrasonic bone knife combined with high-speed turbine minimally invasive surgery for patients with impacted mandibular teeth can not only effectively reduce the patient's pain score,but also maintain the integrity of the extraction socket,reduce bone resorption,and reduce postoperative complications.
