摘要
目的:调查分析黑龙江省县区级结核病定点医疗机构结核病诊断服务能力情况,为全省结核病预防控制服务体系建设提供参考依据。方法:采用横断面调查方法,自拟调查问卷,于2020年8—10月对黑龙江省78家县区级结核病定点医疗机构进行调查,主要了解结核病临床诊疗、胸部影像检查、结核病实验室检测人力资源情况及结核病辅助检查开展情况。结果:78家医疗机构中,从事结核病临床诊疗、胸部影像检查、结核病实验室检测的人员学历以本科及以上为主,占46.7%;职称以初级以下为主,占46.4%。能开展结核病计算机X线摄影(CR)、数字X线(DR)、CT/MRI检查的机构占比分别为16.7%、53.8%、34.6%;能开展结核菌素试验(PPD)、γ干扰素释放试验(IGRA)、抗结核病抗体感染检测、B超检查的机构占比分别为34.6%、2.6%、10.3%、34.6%。全省78家县区级结核病定点医疗机构均未开展支气管镜检查工作。结论:黑龙江省县区级结核病定点医疗机构结核病诊疗专业技术人员相对不足,缺乏高层次人才,结核病实验室诊断能力及结核病影像学检测能力有待提高。
OBJECTIVE To investigate and analyze the tuberculosis diagnosis service ability of county-level tuberculosis designated medical institutions in Heilongjiang province,so as to provide reference for the construction of tuberculosis prevention and control service system in the whole province.METHODS Cross-sectional survey was used to investigate the current situation of 78 county-level designated medical institutions for tuberculosis in Heilongjiang province by self-designed questionnaire,mainly to understand the human resources of clinical diagnosis and treatment of tuberculosis,chest imaging examination,tuberculosis laboratory testing and tuberculosis auxiliary examination in county-level designated medical institutions for tuberculosis.RESULTS Among the 78 county-level designated medical institutions for tuberculosis,46.7%of the personnel engaged in clinical diagnosis and treatment of tuberculosis,chest imaging examination and laboratory detection of tuberculosis had bachelor degree or above.The professional title was below junior level,accounting for 46.4%.Among 78 designated medical institutions for tuberculosis at county level,16.7%,53.8%and 34.6%of the units could carry out computer X-ray photography(CR),digital X-ray(DR)and CT/MRI examination of tuberculosis respectively.Units that can carry out tuberculin test(PPD),interferon-gamma release test(IGRA),anti-tuberculosis antibody infection detection and B-ultrasound examination accounted for 34.6%,2.6%,10.3%and 34.6%,respectively.No bronchoscopy has been carried out in 78 county-level designated medical institutions for tuberculosis in the province.CONCLUSION The designated medical institutions for tuberculosis at county level in Heilongjiang province were short of technical personnel for tuberculosis diagnosis and treatment and high-level talents.The diagnostic ability of tuberculosis laboratory and imaging detection ability of tuberculosis needed to be improved.
