小儿哮喘发作期中医证候与呼出气一氧化氮等炎性标志物的相关性分析 被引量:4

摘要 目的通过研究分析哮喘发作期患儿不同证候类型与呼出气一氧化氮(FeNO)及外周血嗜酸性粒细胞(EOS)绝对值、白细胞计数(WBC)水平的相关性,探索小儿哮喘发作期辨证分型客观化参考依据。方法采用横断面调查研究设计,选择北京中医药大学东直门医院儿科门诊哮喘急性发作期轻、中度患儿,进行中医辨证并测定患儿FeNO及外周血EOS绝对值、WBC水平,进行统计分析。结果1)共纳入患儿47例,其中热性哮喘15例,寒性哮喘9例,外寒内热19例,虚实夹杂4例,各组年龄、性别、过敏史差异无统计学意义(P>0.05)。2)不同证候患儿FeNO水平从高到低依次为寒性哮喘>外寒内热>热性哮喘,其中寒性哮喘组FeNO水平明显高于热性哮喘组;热性哮喘患儿外周血WBC计数明显高于外寒内热,寒性哮喘患儿外周血EOS绝对值明显高于外寒内热,差异均有统计学意义(均P<0.05),其余各证候组比较均无统计学意义(P>0.05)。3)哮喘发作期患儿FeNO水平与外周血EOS绝对值呈弱相关,与WBC计数无相关性(均P>0.05)。结论不同证候哮喘发作期患儿的外周血及气道变应性炎症程度分布存在差异,外周血WBC、EOS及FeNO水平可为小儿哮喘发作期辨证分型提供一定参考依据,这为中医辨证的客观化提供新的方法,并为其后针对性中医治疗提供帮助。 Objective:To explore the objective reference basis of syndrome differentiation in children with asthma attack by studying and analyzing the correlation between different syndrome types and the absolute value of exhaled nitric oxide(FeNO),peripheral blood eosinophil(EOS)and white blood cell(WBC)count.Methods:By using a cross-sectional survey design,mild and moderate children with acute attack of asthma in the pediatric clinic of Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine were selected for TCM syndrome differentiation,and the absolute values of FeNO,EOS and WBC in peripheral blood were measured for statistical analysis.Results:1)A total of 47 children were included,including 15 cases of thermal asthma,9 cases of cold asthma,19 cases of external cold and internal heat,and 4 cases of deficiency and excess.There was no significant difference in age,gender and allergy history among each group.2)The level of FeNO in children with different syndromes from high to low was cold asthma>external cold and internal heat>hot asthma.The level of FeNO in cold asthma group was significantly higher than that in hot asthma group;the WBC count in peripheral blood of children with heat asthma was significantly higher than that of children with external cold and internal heat;the absolute value of EOS in peripheral blood of children with cold asthma was significantly higher than that of children with external cold and internal heat,the difference was statistically significant.There was no statistical significance in the comparison of other syndrome groups.3)The level of FeNO in asthmatic children was weakly correlated with the absolute value of EOS in peripheral blood,but not with the WBC count.Conclusion:There are differences in the distribution of peripheral blood and airway allergic inflammation in children with asthma attack of different syndromes.The levels of WBC,EOS and FeNO in peripheral blood can provide a certain reference basis for syndrome differentiation in children with asthma attack,which p
