摘要
范永升教授认为狼疮性肾炎以肾虚为本,毒、瘀为标,虚实夹杂,可累及心、肺、肝、脾、脑髓诸脏。狼疮性肾炎临床表现多样,中医常见证型有热毒血瘀、湿热内蕴、气阴亏虚、肝肾阴虚、脾肾阳虚等,临证必须坚持辨病(包括辨疾病和辨病理)与辨证结合,灵活施治。本病治则为扶正祛邪,调整阴阳。扶正有益气养阴、滋养肝肾、健脾温肾等治法,祛邪有清热解毒、利水除湿、活血祛瘀、祛风通络等治法。文章从中医病因病机、治法治则、辨病辨证等方面出发,总结范永升教授诊治狼疮性肾炎的学术经验,并列举经典医案加以佐证。
Professor FAN Yong-sheng believes that lupus nephritis based on kidney deficiency, poison, blood stasis as the standard, mixture of virtual and real, can drag in heart, lung, liver, spleen, brain. Lupus nephritis’ s clinical manifestations are diverse. Common syndromes in traditional Chinese medicine include toxic heat and blood stasis, hot and humid embodiment,qi and yin deficiency, liver and kidney yin deficiency, spleen kidney yang deficiency. Clinically, it is necessary to adhere to the combination of disease differentiation(including disease and pathology) and syndrome differentiation, treating flexible. The treatment of this disease is to strengthen vital qi to eliminate pathogenic factor, adjust yin and yang. Nourishing qi and yin,nourishing liver and kidney, strengthening spleen and warming kidney are the main methods to strengthen vital qi, clearing away heat and detoxification, benefiting water and dehumidification, promoting blood circulation and removing blood stasis, dispelling wind and clearing collaterals are the main methods to eliminate pathogenic factor. This paper comprehensively summarizes professor FAN Yong-sheng’s academic experience in the diagnosis and treatment of this disease from the aspects of TCM etiology and pathogenesis, treatment principles and methods, disease differentiation and syndrome differentiation, and one proven case was presented.
