摘要
目的 分析急性冠状动脉综合征(acute coronary syndrome,ACS)合并快速眼动期(rapid eye movement period,REM)阻塞性睡眠呼吸暂停(obstructive sleep apnea,OSA)患者的临床资料特点,并探究其相关危险因素。方法 入选2019年5月至2019年12月于安徽医科大学附属省立医院心内科住院的ACS的患者,所有患者均采用Watch-PAT完成床边睡眠呼吸监测。根据发生在REM的睡眠呼吸低通气指数(apnea hypopnea index during rapid eye movement period,REM-AHI)将患者分为两组,即REM-OSA组(REM-AHI≥15)和非REM-OSA组(REM-AHI<15)。收集两组患者的基线资料,包括一般临床资料、实验室检查以及睡眠监测资料。结果 共计纳入205例患者,其中REM-OSA组151例(73.7%),非REM-OSA组54例(26.3%)。REM-OSA组患者体质量指数(body mass index,BMI)、白细胞总数、血清三酰甘油(triglyceride,TG)及左心室舒张末期内径(left ventricle end-diastolic diameter,LVEDD)均高于非REM-OSA组,差异有统计学意义(均P<0.05)。Logistic回归分析显示TG、LVEDD是ACS患者合并REM-OSA的危险因素。结论 ACS患者合并REM-OSA比例较高。TG、LVEDD是ACS患者合并REM-OSA的危险因素。
Objectives bined with obstructive sleep apnea(OSA)during rapid eye movement(REM)period,and to investigate the risk factors.Methods University from May 2019 to December 2019 were enrolled. All the patients were monitored by bedside sleep respiration using watch-PAT. According to the apnea hypopnea index(AHI)during REM period(REM-AHI),the patients were divided into two groups,namely REM-OSA group(REM-AHI ≥ 15)and non-REM-OSA group(REM-AHI < 15).Base-line data were collected for both groups,including general clinical data,laboratory tests and sleep monitoring.Results (26.3%)in non-REM-OSA group. Body mass index(BMI),total number of white blood cells,serum concentration of triglycerides(TG)and left ventricular end-diastolic diameter(LVEDD)in REM-OSA group were significantly higher than those in non REM-OSA group(all P<0.05). Logistic regression analysis showed that TG and LVEDD were risk factors for REM-OSA in ACS patients.Conclusions LVEDD are risk factors for REM-OSA in ACS patients.
