摘要
目的:探讨脑卒中后癫痫老年患者美国国立卫生研究院卒中量表(NIHSS)评分与血清同型半胱氨酸(Hcy)、尿酸(UA)表达水平的相关性。方法:选择汕头市中心医院2019年9月-2020年11月收治的脑卒中后继发癫痫的老年患者176例为研究组,另选择同期单纯脑卒中老年患者80例为单纯脑卒中组,健康体检志愿者80例为对照组。神经功能缺损程度采用NIHSS评分进行评价。比较各组血清Hcy、UA表达水平,比较研究组中不同NIHSS评分患者血清Hcy、UA表达水平,分析NIHSS评分与血清Hcy、UA表达水平的相关性。结果:研究组血清Hcy、UA表达水平均高于对照组及单纯脑卒中组(P<0.05);重度神经功能缺损组血清Hcy、UA水平高于轻、中度神经功能缺损组,中度神经功能缺损组血清Hcy、UA水平高于轻度神经功能缺损组(P<0.05)。研究组NIHSS评分为(17.11±2.97)分,高于单纯脑卒中组的(14.74±2.16)分,差异有统计学意义(P<0.05)。脑卒中后癫痫患者NIHSS评分与血清Hcy、UA表达水平呈正相关性(r=0.479、0.513,P<0.05)。结论:脑卒中后癫痫老年患者血液中Hcy、UA存在高表达,且与神经功能缺损程度存在密切关系,通过对上述指标进行检测,可为脑卒中后癫痫的预防及预后提供指导。
Objective:To investigate the correlation between National Institutes of Health stroke scale (NIHSS) score and serum homocysteine (Hcy) and uric acid (UA) levels in elderly patients with epilepsy after stroke.Method:A total of 176 elderly patients with epilepsy after stroke in Shantou Central Hospital from September 2019 to November 2020 were selected as the study group,another 80 patients with simple stroke in the same period were selected as the simple stroke group,and 80 elderly healthy volunteers were selected as the control group.The degree of neurological impairment was evaluated by NIHSS score.The expression levels of serum Hcy and UA in each group were compared.The expression levels of serum Hcy and UA in patients with different NIHSS scores in the study group were compared.The correlation between NIHSS score and the expression levels of serum Hcy and UA was analyzed.Result:The expression levels of Hcy and UA in the study group were higher than those in the control group and the simple stroke group (P<0.05);the levels of serum Hcy and UA in severe neurological deficit group were higher than those in mild and moderate neurological deficit group,and the levels of serum Hcy and UA in moderate neurological deficit group were higher than those in mild neurological deficit group (P<0.05).The NIHSS score of the study group was (17.11±2.97) points,which was higher than (14.74±2.16) points of the simple stroke group,and the difference was statistically significant (P<0.05).There was a positive correlation between NIHSS score and serum Hcy,UA expression levels in patients with epilepsy after stroke (r=0.479,0.513,P<0.05).Conclusion:Hcy,UA are highly expressed in the blood of elderly patients with epilepsy after stroke,which are closely related to the degree of neurological impairment.Detection of the above indicators can provide guidance for the prevention and prognosis of post-stroke epilepsy.
