多层螺旋CT三维重建技术在踝关节骨折诊断及术后评估中的临床价值 被引量:2

The Clinical Value of Multi-Slice Spiral CT Three-Dimensional Reconstruction in Diagnosis and Postoperative Evaluation of Ankle Fracture

摘要 目的:探究多层螺旋CT(multi-slice spiral CT,MSCT)三维重建技术(3D reconstruction technique,3D-RT)在踝关节骨折(Ankle Fractures,AF)诊断及术后评估中的临床价值。方法:将2017年7月1日-2020年6月30日中铁十九局集团有限公.司职工中心医院收治的AF患者共100例纳入本次的研究范围,分别在术前、术后进行X线检查和MSCT+3D-RT进行检查,以手术中所见结果作为金标准,对比2种检查方式术前的诊断效能、不同类型AF的检出情况、术后复位质量情况及内固定置入质量情况。结果:MSCT+3D-RT检查诊断AF的准确率、敏感度高于X线检查,差异具有统计学意义(P<0.05);2种检查方式的特异度差异无统计学意义(P>0.05)。MSCT+3D-RT对不同类型AF的术前检出率、术后解剖复位率、内固定质量良好率均高于X线检查,差异有统计学意义(P<0.05)。结论:MSCT+3D-RT在AF诊断及术后评估的应用中具有显著的临床价值,术前能准确诊断疾病类型,术后可客观地评价术后复位和内固定质量,有利于患者的康复。 Objective:To explore the clinical value of multi-slice spiral CT(MSCT)three-dimensional reconstruction technology(3D-RT)in the diagnosis and postoperative evaluation of ankle fractures(AF).Methods:A total of 100 patients with AF treated in the Staff Central Hospital of China Railway 19th Bureau Group Co.,Ltd.from July 1,2017 to June 30,2020 were included in the scope of this study.X-ray examination and MSCT+3D-RT were performed before and after operation respectively.The results seen in the operation were used as the gold standard to compare the preoperative diagnostic efficacy and the detection of different types of ankle fractures,the quality of postoperative reduction and the quality of internal fixation.Results:The accuracy and sensitivity of MSCT+3D-RT in the diagnosis of ankle fracture were higher than those of X-ray examination(P<0.05);There was no significant difference in specificity between the two methods(P>0.05).The preoperative detection rate,postoperative anatomical reduction rate and good quality of internal fixation of different types of ankle fractures by MSCT+3D-RT were higher than those by X-ray examination(P<0.05).Conclusion:MSCT+3D-RT has significant clinical value in the diagnosis and postoperative evaluation of ankle fracture.It can accurately diagnose the type of disease before operation and objectively evaluate the quality of postoperative reduction and internal fixation after operation,which is conducive to the rehabilitation of patients.
