摘要
目的:研究分析短节段经皮椎弓根螺钉固定对胸腰段脊柱爆裂性骨折的临床疗效以及对患者VAS评分的改善。方法:选取2018年12月-2020年1月80例我院收诊胸腰段脊柱爆裂性骨折需行手术患者,并采用随机数字法将患者分为对照组(40例,采用短节段开放椎弓根螺钉固定的手术方式)和观察组(40例,采用短节段经皮椎弓根螺钉固定的手术方式),比较2组患者的围术期指标、手术前后疼痛视觉模拟(VAS)评分以及术后并发症发生率。结果:比较2组患者情况发现,观察组患者手术时间为(62.55+6.48)分钟,较对照组患者的(98.23+11.62)分钟显著降低,且术中出血量为(72.12+18.41)ml,显著少于对照组的术中出血量(226.13+25.36)ml;在术后2组患者疼痛程度都有所减轻,且观察组在术后1天评分为(4.27+1.23)分以及3天评分为(2.45+0.55)分,较对照组患者(6.52+1.02分;4.33+0.89分)明显减轻;与此同时,观察组的术后并发症发生率为7.50%,也显著低于对照组患者的30.00%(P<0.05)。结论:在对于胸腰段脊柱爆裂性骨折患者的手术治疗中,采用短节段经皮椎弓根螺钉固定的方式能够缩短患者的手术时间,并减少患者术中出血量,同时显著减轻了患者的疼痛程度,术后并发症发生率小,具有较高的安全性。
Objective:To study and analyze the clinical efficacy of short-segment percutaneous pedicle screw fixation for thoracolumbar burst fractures and the improvement of patients´VAS scores.Methods:80 cases of thoracolumbar burst fractures requiring surgery were selected in our hospital,and the period was from December2018 to January 2020.The patients were divided into a control group(40 cases,using short-segment open pedicle screw fixation)and a study group(40 cases,using short-segment percutaneous pedicle screw fixation surgery),to compare the perioperative indicators,visual simulation of pain(VAS)scores before and after surgery,and the inci-dence of postoperative complications between the two groups.Results:Comparing the two groups of patients,it was found that the operation time of the study group(62.55±6.48)was significantly lower than that of the control group(98.23±11.62),and the intraopera-tive blood loss(72.12±18.41)was significantly less than that of the control group.The amount of pain in the two groups was reduced af-ter surgery,and the pain in the study group at 1 day(4.27±1.23)and 3 days(2.45±0.55)after surgery was higher than that of the control group(6.52)±1.02;4.33±0.89)was significantly reduced;at the same time,the postoperative complication rate of the study group(7.50)was also significantly lower than that of the control group(30.00)(P<0.05).Conclusion:In the case of thoracolumbar spine In the surgical treatment of patients with burst fractures,the use of short-segment percutaneous pedicle screw fixation can shorten the operation time of the patient,reduce the amount of bleeding during the operation,and significantly reduce the pain of the patient and postoperative complications The incidence is small and has high safety.
