摘要
目的比较不同肥胖指标对2型糖尿病(T2DM)患者发生阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的筛查价值。方法监测157例住院的T2DM患者睡眠呼吸情况,依据睡眠呼吸暂停低通气指数(AHI)将患者分为非OSAHS组和OSAHS组。比较体重指数(BMI)、颈围(NC)、腰围(WC)、腰臀比(WHR)、腰围身高比(WHtR)、人体肥胖指数(BAI)、中国人内脏脂肪指数(CVAI)和内脏脂肪面积(VFA)8个肥胖指标在T2DM患者中筛查OSAHS的能力,分析其可行性。结果非OSAHS组41例(26.1%),OSAHS组116例(73.9%)。在男性和女性患者中OSAHS组的肥胖指标均明显高于非OSAHS组,且差异均有统计学意义(P<0.01);Pearson分析显示无论男女8个肥胖指标与OSAHS的严重程度均呈正相关,其中VFA、CVAI、WHtR和WC的相关性更高;ROC曲线下面积(AUC)显示8个肥胖指标对OSAHS均有一定的诊断能力,其中诊断价值较好的(AUC>0.9)为VFA、CVAI、WHtR和WC,AUC最大者为VFA,在男性切点为97.5 cm^(2)(敏感性99.4%,特异性100%);女性为99.5 cm^(2)(敏感性88.9%,特异性100%)。结论以上肥胖指标可用于在T2DM患者中筛查OSAHS,其中VFA筛查OSAHS的敏感度和特异度最高。故在T2DM患者中早期测量VFA用于筛查OSAHS,但在无条件进行VFA测量时,对T2DM患者进行WHtR和WC的测量,也可用于OSAHS的筛查,以期早期防治。
Objective To investigate the screening value of different obesity indexes for the occurrence of obstructive sleep apnea hypopnea syndrome(OSAHS)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 157 patients with T2DM were enrololed in the study.According to sleep apnea hypopnea index(AHI),the patients were divided into non-OSAHS group and OSAHS group.The ability of body mass index(BMI),neck circumference(NC),waist circumference(WC),waist to hip ratio(WHR),waist circumference height ratio(WHtR),human obesity index(BAI),Chinese visceral fat index(CVAI)and visceral fat area(VFA)in screening OSAHS were observed and compared between the two grosup.Results There were 41 cases(26.1%)in non-OSAHS group and 116 cases(73.9%)in OSAHS group.The obesity indexes in OSAHS group were significantly higher than those in non-OSAHS group(P<0.01).Pearson showed that the eight obesity indexes were positively correlated with the severity of OSAHS in both men and women,in whch,the correlation of VFA,CVAI,WHtR and WC was higher.The area under ROC curve(AUC)showed that the eight obesity indexes had a certain diagnostic efficacy for OSAHS,and the diagnostic value of VFA,CVAI,WHtR and WC,AUC was better than that of VFA,and the cutoff value of male was 97.5cm^(2)(sensitivity 99.4%,specificity 100%),and female was 99.5cm^(2)(sensitivity 88.9%,specificity 100%).Conclusion The obesity indexes mentioned above can be used to screen OSAHS in patients with T2DM,in which,VFA has the highest sensitivity and specificity.Therefore,early detection of VFA is beneficial for screening OSAHS,and for early prevention and treatment of OSAHS.
