摘要
目的利用静息态功能磁共振成像(resting-state functional magnetic resonance imaging,rs-fMRI)技术功能连接和局部一致性(regional homogeneity,ReHo)的方法探讨梅尼埃病患者静息态脑活动的变化。材料与方法选取梅尼埃病发作期患者12例和健康对照组12例,进行rs-fMRI扫描,基于REST-plus软件,以双侧丘脑为感兴趣区,比较两组脑功能连接差异;运用肯德尔一致系数局部一致性(Kendall′s coefficient concordance regional homogeneity,KCC-ReHo)和相干性局部一致性(coherence regional homogeneity,Cohe-ReHo)两种方法分析数据,比较两组受试者的ReHo值。结果梅尼埃病患者组双侧丘脑和右侧额下回功能连接增强;KCC-ReHo增高的脑区主要位于左侧颞下回,减低的脑区主要位于右侧额上回;Cohe-ReHo增高的脑区主要位于右侧小脑、左侧颞下回,减低的脑区主要位于右侧额上回、右顶上回、右侧楔前叶(Alphasim校正,校正后P<0.05)。结论静息状态下梅尼埃病患者脑功能连接和ReHo值均发生改变,主要涉及中枢前庭系统和视觉皮层相关脑区,可能与梅尼埃病的发病机制有关。
Objective:To investigate the alterations of functional connectivity(FC)and regional homogeneity(Re Ho)by the resting-state functional magnetic resonance imaging(rs-f MRI)in patients with Ménière disease(MD).Materials and Methods:A total of 12 patients with active MD and 12 healthy control subjects were enrolled to rs-f MRI scan.Seed-based functional connectivity analysis of bilateral thalamus was performed using the REST-plus software.The data were analyzed to evaluate the differences in Re Ho values between the two groups by the Kendall’s coefficient concordance regional homogeneity(KCC-Re Ho)and coherence regional homogeneity(Cohe-Re Ho).Results:Compared with the healthy control group,the increased connectivity of right thalamus and right inferior frontal gyrus was showed in MD patients.KCC-Re Ho in MD patients were higher in the left inferior temporal gyrus,lower in the right superior frontal gyrus;Cohe-Re Ho in MD patients was higher in the right cerebellum and the left inferior temporal gyrus,lower in the right parietal superior gyrus,right superior frontal gyrus,and left precuneus(corrected by Alphasim,P<0.05 after correction).Conclusions:This study identified specific alterations of FC and Re Ho values in patients with MD,which may relate to the pathogenesis of Meniere’s disease.
