精细化营养支持对食管癌患者术后营养状况、恢复情况及生活质量的影响 被引量:12

Effect of refined nutritional support on postoperative nutritional status, recovery and quality of life in patients with esophageal cancer

摘要 目的 探讨精细化营养支持对食管癌患者术后营养状况、恢复情况及生活质量的影响。方法 按照随机数字表法将120例食管癌根治术患者分为对照组(n=60)和观察组(n=60),对照组患者给予常规干预,观察组患者在对照组的基础上给予精细化营养支持。比较两组患者的营养指标[白蛋白(ALB)、前白蛋白(PA)、血红蛋白(Hb)]、术后恢复指标、术后并发症发生情况及生活质量。结果 干预后,两组患者的ALB、PA、Hb水平均高于本组干预前,且观察组患者的ALB、PA、Hb水平均高于对照组,差异均有统计学意义(P﹤0.05)。观察组患者术后首次肛门排气时间、首次排便时间、肠鸣音恢复时间及首次进食时间均明显短于对照组(P﹤0.01)。观察组患者术后吻合口狭窄和功能性胃排空障碍的发生率均低于对照组(P﹤0.05);两组患者反流性食管炎、严重腹泻和吻合口瘘的发生率比较,差异均无统计学意义(P﹥0.05)。干预后,两组患者生活质量评定量表(QOLS)中心理功能、身体功能、生理功能和社会功能评分均高于本组干预前,且观察组患者的上述评分均高于对照组,差异均有统计学意义(P﹤0.05)。结论 精细化营养支持可改善食管癌患者术后营养状况,缩短术后恢复时间,对减少并发症和提高患者生活质量具有积极意义。 Objective To explore the effect of refined nutritional support on postoperative nutritional status, recovery and quality of life in esophageal cancer patients. Method A total of 120 patients undergoing esophagectomy were divided into control group (n=60, received routine intervention) and observation group (n=60, received additional refined nutrition support based on routine intervention) according to random number table. The nutritional indicators [albumin (ALB),prealbumin (PA) and hemoglobin (Hb)], postoperative recovery indicators, postoperative complications and quality of life were compared between the two groups. Result After intervention, the levels of ALB, PA and Hb in the two groups were significantly higher than those before intervention, with higher levels in the observation group than those in the control group (P<0.05). The first anal exhaust time, first defecation time, bowel sound recovery time and first feeding time after surgery in the observation group were significantly shorter than those in the control group (P<0.01). The incidence of anastomotic stenosis and functional gastric emptying disorder in observation group were significantly lower than those in control group (P<0.05). There were no significant differences in the incidence of reflux esophagitis, severe diarrhea and anastomotic fistula between the two groups (P>0.05). After intervention, the scores of psychological function, physical function, physiological function and social function in quality of life scale (QOLS) in two groups were significantly higher than those in the same group before intervention, with higher scores in observation group than those in control group (P<0.05). Conclusion Refined nutritional support can obviously improve the nutritional status, shorten the recovery time, reduce the occurrence of postoperative complications and improve the quality of life of patients with esophageal cancer.
