摘要
目的探讨徐州地区儿童肺炎克雷伯菌血流感染(KPBSI)特征,为临床药物治疗提供依据。方法回顾性分析江苏省徐州市儿童医院2015年1月至2021年7月KPBSI患儿临床资料、实验室感染性指标及药敏试验结果。结果76例KPBSI患儿中脓毒血症19例、肺炎17例;伴有发热50例、咳嗽15例。外周血白细胞(WBC)计数、C反应蛋白、降钙素原的增高占比分别是63.2%、95.1%、87.0%。耐碳青霉烯类肺炎克雷伯菌(CRKP)检出16株(21.1%),碳青霉烯敏感肺炎克雷伯菌检出60株(78.9%)。CRKP对所有头孢菌素、氨曲南、β-内酰胺酶抑制剂复合制剂都具有高耐药率。检出CRKP的患儿中,新生儿多,实验室指标多异常,临床症状多明显。结论CRKP患儿对常用药物具有高耐药率,其临床特征明显。
Objective To investigate the characteristics of Klelbsiela pneumoniae bloodstream infection(KPBSI)of children in Xuzhou area,in order to provide basis for clinical drug therapy.Methods Clinical and laboratory infectivity indexes,and drug sensitivity tests of children with KPBSI from January 2015 to July 2021 in Xuzhou Children’s Hospital of Jiangsu Province were retrospectively analyzed.Results There were 19 cases of sepsis and 17 cases of pneumonia in 76 children with KPBSI,and 50 cases with fever,15 cases with cough.The percentages of peripheral blood white blood cell(WBC),C-reactive protein,and procalcitonin increased,63.2%,95.1%,and 87.0% respectively.16 strains(21.1%)of carbapenem-resistant Klebsiella pneumoniae(CRKP)were detected,and 60 strains(78.9%)of CRKP were detected.CRKP has a high drug resistance rate to all cephalosporin,amtronam andβ-lactamase inhibitor compounds.Most of the children with CRKP were neonates,abnormal laboratory indicators and obvious clinical symptoms.Conclusion CRKP children have high drug resistance rate to commonly used drugs,and its clinical characteristics are obvious.
