摘要
目的 分析进食评估问卷调查工具-10 (EAT-10)联合容积-黏度测试(V-VST)在老年急性缺血性脑卒中(AIS)后吞咽功能障碍筛查中的效能。方法 2018年3月-2021年3月在徐州市中心医院康复科诊治的98例老年AIS患者,进行卒中后吞咽功能障碍筛查,以视频透视吞咽功能检查(VFSS)方法为标准,采用Kappa一致性检验分析比较EAT-10和V-VST单独筛查和联合筛查的灵敏度、特异度和准确度。结果 98例AIS患者经VFSS检查确诊51例吞咽功能障碍患者,EAT-10筛查共检出42例阳性,检出率为42.86%,V-VST筛查共检出47例阳性,检出率为47.96%,EAT-10联合V-VST筛查共检出48例阳性,检出率为48.98%;与VFSS检查标准诊断相比,Kappa值分别为0.614、0.796、0.857;EAT-10、V-VST筛查以及两者联合筛查AIS后吞咽功能障碍的敏感度分别为72.55%、78.43%、90.20%,特异度分别为89.36%、93.62%、95.74%,准确率分别为80.61%、89.79%、92.86%。讨论EAT-10联合V-VST筛查AIS后吞咽功能障碍,可有效提高检测灵敏度和特异性,降低漏诊率及误诊率。
Objective To analyze the efficiency of Eating Assessment Tool-10(EAT-10) combined with Volume-Viscosity Swallow Test(V-VST) in screening swallowing dysfunction in the elderly after acute ischemic stroke(AIS).Methods A total of 98 elderly patients with AIS treated in Xuzhou Central Hospital Rehalilitation Department were enrolled between March 2018 and March 2021.All were screened for post-stroke swallowing dysfunction.Taking Video Fluoroscopic Swallow Study(VFSS) as the golden standard,sensitivity,specificity and accuracy of EAT-10,V-VST and their combination were compared by Kappa consistency test.Results In the 98 patients with AIS,VFSS showed that there were 51 cases with swallowing dysfunction.EAT-10,V-VST and their combination showed that there were 42 positive cases(detection rate of 42.86%),47 positive cases(detection rate of 47.96%) and 48 positive cases(detection rate of 48.98%),respectively.Compared with the golden standard of VFSS diagnosis,Kappa values of EAT-10,V-VST and their combination were 0.614,0.796 and 0.857,respectively.The sensitivity,specificity and accuracy of EAT-10,V-VST and their combination for screening swallowing dysfunction were(72.55%,78.43%,90.20%),(89.36%,93.62%,95.74%) and(80.61%,89.79%,92.86%),respectively.Conclusion EAT-10 combined with V-VST in the screening of swallowing dysfunction after AIS can effectively improve detection sensitivity and specificity,reduce missed diagnosis and misdiagnosis rates.
