摘要
肺复张广泛应用于急性呼吸窘迫综合征(Acute Respiratory Distress Syndrome,ARDS)的治疗,呼气末正压(Positive End-Expiratory Pressure,PEEP)的应用是保障肺复张效果的关键要素。为患者选择适宜的PEEP能够改善肺复张的治疗效果,并在通气过程中保护肺泡。用传统方法评价肺复张的效果缺乏客观性和准确性,而影像学在观察基础疾病的同时还可进行定量评价,其对肺复张效果的评价最直观。精准医疗需求的加大使得运用影像学评价肺复张效果以帮助临床选择适宜PEEP成为目前研究的热门方向。本文对CT、超声和电阻抗断层成像评价ARDS治疗中呼气末正压通气的PEEP值选择的研究进展进行了综述。
Lung recruitment is widely used in the treatment of acute respiratory distress syndrome(ARDS).The application of positive end-expiratory pressure(PEEP)is the key factor to ensure the effect of lung reexpansion.Patients’selection of appropriate PEEP can improve the effect of lung reexpansion and protect alveoli during ventilation.Traditional methods to evaluate the effect of lung reexpansion lack objectivity and accuracy,while imaging can quantitatively evaluate lung reexpansion effect while observing the underlying diseases,which is the most intuitive evaluation of lung reexpansion effect.The increasing demand for precision medicine has made how to use imaging to evaluate the lung reexpansion effect and help clinical select optimal PEEP become the current research direction.In this review,we summarized the research progress referring to the evaluation of PEEP selection in ARDS by CT,ultrasound and electrical impedance tomography(EIT).
