摘要
目的对比评估功能性踝关节不稳(FAI)者在不同跑速落地阶段踝关节的肌电激活模式,并评估动力学约束对踝关节动态稳定性的影响。方法招募14名功能性踝关节不稳(FAI)男青年和14名年龄、性别、身高、体重相匹配的无FAI的对照组,在跑步机上以自选跑速、快速、慢速在跑步机上跑5min,通过三维动作捕捉收集运动学数据和下肢肌电数据,计算跑步各个阶段下肢胫骨前肌(TA)/腓骨长肌(PL)、胫骨前肌(TA)/腓肠肌外侧头(GL)、胫骨前肌(TA)/比目鱼肌(TS)的肌肉共收缩(CII)指数。采用独立样本t检验进行组间比较。结果两组在自选跑速下TA/PL的CII在着地后100~200ms存在差异(p<0.05),在其他阶段无差异;两组TA/GM的CII在阶段Ⅰ及阶段Ⅳ的存在差异(p<0.01);FAI组与对照组的TA/TS的CII指数在阶段Ⅱ存在差异(p<0.01),在其他任何着陆阶段均无差异。较快跑速下FAI组与对照组阶段Ⅳ的TA/GM的CII存在差异(p<0.01);TA/PL的CII指数在阶段Ⅳ存在差异(p<0.05)。较慢跑速下两组的TA/PL在整个跑动过程中的任何着陆阶段均无差异(p>0.05)。两组在阶段Ⅲ的TA/GM的CII存在差异(p<0.05);TA/TS的CII指数在阶段Ⅳ存在差异(p<0.05)。结论两组在额状面及矢状面的肌肉共收缩模式相似,FAI患者采用主动约束的策略来稳定踝关节。但FAI组的CII仍然大于对照组,FAI着陆策略的无效可能是反馈神经肌肉控制缺陷的结果。
Objective To evaluate the EMG activation patterns of ankle joints in patients with and without functional ankle instability(FAI)at different running speeds and landing stages,and to evaluate the effect of dynamic constraints on ankle dynamic stability.Methods Fourteen young men with functional ankle instability(FAI)and 14 control subjects without FAI were recruited to run on a treadmill for 5 minutes at a self-selected running speed,fast and slow.Kinematic data and lower limb emG data were collected by 3D motion capture.Co-contraction index(CII)of tibialis anterior muscle(TA)/peroneus longus muscle(PL),tibialis anterior muscle(TA)/gastrocnemius lateral head(GL)and tibialis anterior muscle(TA)/soleus muscle(TS)were calculated in each running stage.The independent sample T test was used to compare groups.Results The CII of TA/PL was different between the two groups at the free running speed at 100-200ms after landing(p<0.05),but there was no difference at other stages.The CII of TA/GM in stageⅠand stageⅣwere different between the two groups(p<0.01).The CII index of TA/TS in FAI group and control group was different at stageⅡ(p<0.01),and there was no difference at any other landing stage.The CII of TA/GM in FAI group was different from that in control group(p<0.01).The CII index of TA/PL was different in stageⅣ(p<0.05).There was no difference in TA/PL at any landing stage during the whole running process between the two groups compared with the jogging speed(p>0.05).There was difference in TA/GMCII between the two groups in stageⅢ(p<0.05).The CII index of TA/TS was different in stageⅣ(p<0.05).Conclusion The co-contraction patterns of frontal and sagittal muscles were similar in the two groups,and active restraint was used to stabilize the ankle joint in FAI patients.However,the CII of the FAI group was still larger than that of the control group.The ineffectiveness of the FAI landing strategy may be the result of feedback neuromuscular control defects.
