摘要
目的分析肝细胞癌骨转移(hepatocellular carcinoma bone metastasis,HCC-BM)的临床及病理学特征,检测脱嘌呤/脱嘧啶核酸内切酶1(apurinic/apyrimidinic endonuclease-1,APEX1)和JAG1基因在HCC-BM和肝细胞癌(hepatocellular carcinoma,HCC)中的表达;探讨两者的表达与临床病理特征的关系。方法回顾性分析2012年1月—2019年12月在福建医科大学附属第一医院和福建医科大学附属第二医院HCC-BM患者的资料。采用免疫组化检测79例HCC及HCC-BM中APEX1、JAG1的表达,分析两者与临床病理特征的关系。结果福建医科大学附属第一医院和第二医院2012—2019年间肝细胞癌骨转移的发生率为17.02%(79/464)。患者男女比例8.9︰1;中位年龄53岁。脊柱(72.0%)为最常见的骨转移部位,其次为下肢骨11.0%。免疫组化结果显示79例HCC-BM中APEX1胞质、JAG1的阳性率分别为63.3%、62.0%;HCC中的阳性率分别为46.8%、41.8%。APEX1、JAG1在HCC-BM中的表达高于HCC;APEX1的表达与肿瘤的分化程度、原发灶的数目有关(P<0.05);JAG1的表达与肿瘤分化程度、是否合并骨外转移有关(P<0.05)。在HCC-BM中APEX1和JAG1蛋白之间的表达呈正相关(P=0.035)。结论HCC-BM的发生率为17.02%,好发于中老年男性,脊柱为最常见的骨转移部位。APEX1及JAG1在HCCBM中的表达高于HCC,提示两者可能参与肝细胞癌的骨转移,而其具体作用机制尚有待进一步研究。
Objective To analyze the clinical and pathological characteristics of hepatocellular carcinoma bone metastasis(HCCBM)patients and investigate the expression of apurinic/apyrimidinic endonuclease-1(APEX1)and JAG1 in primary hepatocellular carcinoma(HCC)and hepatocellular carcinoma bone metastasis(HCCBM).Methods The data of HCC-BM patients in the First Affiliated Hospital of Fujian Medical University and Second Affiliated Hospital of Fujian Medical University from January 2012 to December 2019was retrospectively reviewed.Immunohistochemistry was used to detect the expression of APEX1 and JAG1 in 79 patients of HCC and HCC-BM,and the correlation with clinicopathologic features was analyzed.Results The incidence of bone metastasis of hepatocellular carcinoma was 17.02%(79/464)from the First and Second Affiliated Hospital of Fujian Medical University.Patients with male and female ratio was 8.9:1.The median age was 53.The common sites of bone metastasis were vertebra(72.0%),followed by lower extremity(11.0%).Immunohistochemical results showed that the positive rate of APEX1and JAG1 in 79 cases of HCC-BM was 63.3%and 62.0%,the positive rates of HCC were 46.8%and 41.8%.The expression of APEX1 and JAG1 in HCC-BM was higher than those in HCC.The expression of APEX1 was related to the degree of tumor differentiation and the number of primary lesion(P<0.05);the expression of JAG1 was related to the degree of tumor differentiation and whether there was extracosseous metastasis(P<0.05).There was a positive correlation between the APEX1 and JAG1protein in HCC-BM(P=0.035).Conclusion The incidence of HCC-BM is 17.02%,which is mostly found in middle-aged and elderly men,and the vertebra is the most common site of bone metastasis.The expression of APEX1 and JAG1in HCC-BM is higher than that of HCC,suggesting that the two genes may be involved in bone metastasis of hepatocellular carcinoma,and their specific mechanism of action remains to be further studied.
