摘要
目的观察血清分泌型蛋白Dikkopf1(DKK-1)、乳酸脱氢酶(LDH)在结直肠癌(CRC)患者中的表达,并分析二者与患者腹腔镜手术治疗预后的关系。方法选取2019年2月至2020年10月河南科技大学第一附属医院收治的106例CRC患者为研究对象。患者均接受腹腔镜手术治疗。术后随访1a,记录局部复发或远处转移情况,并将局部复发或远处转移情况作为预后观察指标。于术前检测患者血清DKK-1、LDH水平及其他实验室指标,包括血清癌胚抗原(CEA)、糖类抗原19-9(CA19-9)。询问并记录患者基线资料,分析各指标与CRC患者腹腔镜手术治疗预后的关系。结果随访1a内,106例CRC患者中2例失访,最终纳入104例患者。104例患者中发生局部复发或远处转移24例(23.08%),纳入预后不良组,未发生局部复发或远处转移80例(76.92%),纳入预后良好组。预后不良组血清DKK-1水平低于预后良好组,血清CEA、CA19-9、LDH水平均高于预后良好组(P<0.05)。logistic回归分析结果显示,血清DKK-1低表达以及CEA、CA19-9、LDH过表达与CRC患者腹腔镜手术治疗后预后不良有关(P<0.05)。结论血清DKK-1低表达、LDH过表达与CRC患者腹腔镜手术治疗后预后不良存在一定的联系。
Objective To observe the expression of serum secretory protein Dikkopfl(DKK-1)and lactate dehydrogenase(LDH)in patients with colorectal cancer(CRC),an to analyze the relat ionship between them and the prognosis of patients after laparoscopie surgery.Methods A total of 106 patients with CRC in the First Affiliated Hospital of Henan University of Science and Technology from February 2019 to October 2020 were selected as the research subjects.All patients underwent laparoscopic surgery.The patients were fol lowed up forl year,and the local recurrence or distant metastasis were recorded,and local recurrence or distant metastasis was used as prognostic indicators.The levels of serum DKK-1,LDH and other laboratory indexes including serum carci noembryonie antigen(CEA),carbohydrate antigen 19-9(CA19-9)of patients were measured before operation.The baseline data of patients were asked and recorded.The rel at ionship between each index and the prognosis of CRC patients afer laparoscopic surgery was analyzed.Results Within one year of follow-up,2 of 106 CRC patients were lost,and 104 patients were finally included.Among 104 patients,24 cases(23.08%)had local recurrence or distant metastasis,who were included in poor prognosis group,while 80 cases(76.92%)had no local recurrence or distant metastasis,who were incuded in good prognosis group.The level of serum DKK-1 in poor prognosis group was lower than that in good prognosis group,while the levels of serum CEA,CA19-9 and LDH were higher than those in good prognosis group(P<0.05).Logistie regression analysis showed that the low expression of serum DKK-1 and the overexpression of CEA,CA19-9 and LDH.were related to the poor prognosis of CRC patients after laparoscopic surgery(P<0.05).Conclusion The low expression of serum DKK-1 and overexpression of LDH are related to the poor prognosis of CRC patients after laparoscopic surgery.
