职业性油雾暴露与肺功能损伤的调查 被引量:2

Investigation on occupational oil mist exposure and lung function injury

摘要 目的明确油雾(金属加工液)职业暴露与肺功能损伤发生的关系,探究不同程度油雾暴露与肺功能损伤发生之间的剂量-反应关系。方法收集2019年1—12月在无锡市第八人民医院进行职业健康检查的某单位2631名在岗员工的健康检查资料,同时进行职业健康相关方面的普查,统计分析职业性油雾暴露与肺功能损伤之间的关系。结果2631名调查对象中肺部影像学结果异常者204人,占7.8%;肺功能异常者270人,占10.3%。油雾暴露组与对照组的肺功能损伤及肺部影像学异常率比较,差异均有统计学意义(均P<0.05)。油雾职业暴露是肺部影像学异常的重要危险因素,接触该物质会导致肺部影像学结果异常率上升(OR=1.466,95%CI:1.048~2.051,P<0.05),同时,该有害物质也是肺功能异常的危险因素,油雾职业暴露会导致肺功能异常率上升(OR=2.131,95%CI:1.545~2.940,P<0.01)。结论油雾职业暴露是肺部影像学不良结局与肺功能损伤的危险因素,应加强对具备该特征人群的关注,以免出现严重的肺部损伤结局。 Objective To clarify the relationship between occupational exposure to oil mist(metal processing fluid)and pulmonary function injury,explore the dose-response relationship between different degrees of oil mist exposure and lung function injury.Methods The physical examination data of 2631 employees who underwent occupational health examination in the Wuxi Eighth People’s Hospital from January to December 2019 were collected,at the same time,the general survey of occupational health was carried out.The correlation between occupational oil mist exposure and lung function injury was analyzed.Results Among 2631subjects,204 had abnormal pulmonary imaging results,accounting for 7.8%.There were 270 patients with abnormal pulmonary function,accounting for 10.3%.There were statistically significant differences in the lung function injury and lung imaging between oil mist exposure group and control group(all P<0.05).Occupational oil mist exposure was an important risk factor for abnormal lung imaging results,and it may lead to an increased abnormal rate of lung imaging results(OR=1.466,95%CI:1.048-2.051,P<0.05).At the same time,this harmful substance was also a risk factor for abnormal lung function,and it may lead to an increased rate of abnormal lung function(OR=2.131,95%CI:1.545-2.940,P<0.01).Conclusion Oil mist is a risk factor for adverse outcome of lung imaging and lung function injury,and more attention should be paid to the population with this characteristic to avoid serious outcome of lung injury.
