摘要
目的探究电子化认知功能评价系统(computerized neuropsychological assessment device,CNAD)对无症状颈动脉重度狭窄(severe asymptomatic carotid artery stenosis,SACAS)的筛查价值。方法从2020年9月至2021年8月在首都医科大学附属北京安贞医院血管外科连续招募SACAS患者(狭窄组)和无狭窄受试者(对照组)进行前瞻性研究,通过CNAD被定量测量并比较两组患者的认知功能,通过受试者工作特征曲线评价CNAD的筛查价值。结果共纳入44例SACAS患者和47例无颈动脉狭窄患者。狭窄组患者的色词干扰测试(P=0.012)和鉴别测试(P=0.013)评分较差。线性回归方程分析显示,狭窄组患者的认知评分随年龄的增长下降较快,尤其是数字广度测试和鉴别测试。色词干扰测试(P=0.008)、鉴别测试(P=0.009)及两者的综合指数(P=0.002)对无症状颈动脉重度狭窄患者具有筛查价值。结论SACAS患者的认知功能低于无颈动脉狭窄者,特别是在注意力和执行能力方面。CNAD对SACAS患者具有一定的筛查价值。
Objective To explore the value of computerized neuropsychological assessment device(CNAD)in screening patients with severe asymptomatic carotid artery stenosis(SACAS).Methods SACAS patients(SACAS group)and patients without carotid stenosis(control group)were continuously recruited in the Department of Vascular surgery,Beijing Anzhen Hospital,Capital Medical University from September 2020 to August 2021 for prospective research.The cognitive function of the two groups was quantitatively measured and compared by CNAD.The diagnostic value of CNAD was evaluated by the receiver operating characteristic(ROC)curve.Results 44 SACAS patients and 47 patients without carotid stenosis were recruited.SACAS group had worse scores for Stroop color-word test(P=0.032)and identification test(P=0.040)than that of control group.The analysis of linear regression equation indicated that cognitive scores of stenosis patients declined faster with age,especially for digit span test and identification test.The Stroop color-word test,identification test and comprehensive index of the two tests had the value of SACAS screening.Conclusion The cognitive function of SACAS patients is lower than that of patients without carotid stenosis,especially corresponding to attention and executive ability.The CNAD has screening value for patients with SACAS.
