摘要
目的 探讨先天性外耳道畸形伴胆脂瘤的临床特点及预后。方法 收集2013年8月-2019年10月于福建医科大学附属第一医院手术的先天性外耳道畸形伴胆脂瘤19例(20耳)患者的临床资料,复习相关文献,分析其临床特点及预后。结果 本组病例中,发病年龄为2-30岁,平均9.4岁。主要表现为听力下降(20/20)、耳痛(17/20)、耳漏(12/20)、面瘫(2/20)。胆脂瘤位于外耳道骨部16耳,外耳道及中耳3耳,外耳道软骨部1耳。对本组患者进行2-75个月随访,发生耳道再狭窄6耳,胆脂瘤复发1耳。结论 当先天性外耳道畸形伴有耳痛、耳漏等表现时要高度怀疑其伴发胆脂瘤。胆脂瘤多见于外耳道骨部。一旦确诊胆脂瘤,应尽早手术。重建并尽量扩大耳道及术后耳道支撑以减少术后再狭窄。
OBJECTIVE To investigate the clinical features and prognosis of congenital external auditory canal malformation with cholesteatoma.METHODS Thisstudy presents a retrospective review and follow-up of 19 patients(20 ears) with congenital aural deformity accompanied by ear canal cholesteatoma which underwent surgery at First Affiliated Hospital of Fujian Medical University from August 2013 to October 2019.Review of relevant literature was made and clinical features and prognosis were summarized.RESULTS The mean age at surgery was 9.4 years(range:2-30 years).Hearing loss was noted in 20(100%) patients,otalgia in 17(85%),otorrhea in 12(60%) and facial nerve paralysis in 2(10%).Cholesteatoma arose from bony auditory canal in 16 ears,cartilaginous auditory canal in 1 ear and external ear canal and middle ear in 3 ears.All patients were followed up for 2-75 months.Six ears had recurrent aural stenosis and 1 ear had cholesteatoma recurrence.CONCLUSION Clinicians should highly suspect cholesteatoma when otalgia,otorrhea occurred in these patients.Cholesteatoma usually arises in bony auditory canal.Surgery is effective and should be performed as early as possible.A large reconstructed canal and external auditory canal stent prevents postoperative ear canal stenosis.
