摘要
目的探索高压氧治疗脑损伤合并视网膜病变婴幼儿的眼底变化。方法54例脑损伤合并视网膜病变婴幼儿,均进行高压氧疗法治疗。分析高压氧治疗前、中、后眼底筛查结果和高压氧治疗后电话随访结果。结果高压氧治疗前眼底筛查:单侧早产儿视网膜病变(ROP)3例,双侧ROP 17例,视网膜冷凝术后11例;高压氧治疗中和治疗后眼底筛查(10、20、30次各复查1次):正常10例,稳定12例,好转4例,黄斑病变3例,右眼视网膜出血1例,死亡1例。2019年12月电话随访结果:28例眼科检查正常,2例黄斑病变患儿(双胞,男,5岁7月龄)家长自诉正常(拒绝眼科复查),另1例黄斑病变患儿失联,无新的视网膜病变诱发病例。结论脑损伤合并视网膜病变婴幼儿采用高压氧治疗前后未发现加重视网膜病变,也未见诱发视网膜病变新病例,鉴于样本量不大,有待于扩大研究进一步验证其高压氧治疗的安全性。
Objective To investigate the fundus follow-up results of hyperbaric oxygen therapy in infants with brain injury and retinopathy.Methods 54 infants with brain injury and retinopathy were treated with hyperbaric oxygen therapy.The results of fundus screening before,during and after hyperbaric oxygen therapy,and the results of telephone follow-up after hyperbaric oxygen therapy were analyzed.Results Fundus screening before hyperbaric oxygen therapy:3 cases of unilateral retinopathy of prematurity(ROP),17 cases of bilateral ROP,11 cases after retinal condensation;fundus screening during and after hyperbaric oxygen therapy(1 review each at 10,20 and 30 times):10 normal cases,12 stable cases,4 improving cases,3 cases of macular degeneration,1 case of retinal hemorrhage in the right eye,and 1 case of death.Results of telephone follow-up in December 2019:28 cases of ophthalmological examination were normal,2 cases of macular degeneration(twin,male,5 years and 7 months old)were reported by their parents to be normal(refusing ophthalmological review),and the other 1 case of macular degeneration was lost to follow-up,with no new retinopathy-induced cases.Conclusion No exacerbation of retinopathy are found before and after hyperbaric oxygen therapy or new cases of induced retinopathy in infants with brain injury and retinopathy.In view of the small sample size,an expanded study is needed to further verify the safety of their hyperbaric oxygen therapy.
