摘要
目的探究纳洛酮联合无创呼吸机对慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者肺功能及动脉血氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、酸碱度(pH)水平的影响。方法92例研究对象均于浦北县人民医院治疗的COPD合并呼吸衰竭患者中选择,时间为2020年3月至2021年3月,根据随机数字表将其分为对照组和试验组,各46例。两组入院进行常规治疗,对照组再应用无创呼吸机治疗,试验组在对照组的基础上,给予纳洛酮治疗。均给予两组连续治疗3 d。比较两组治疗后临床疗效、治疗前后肺功能、炎性因子及血气指标。结果治疗后试验组总有效率(93.48%)较对照组(78.26%)高;两组治疗后用力肺活量(FVC)、呼气高峰流量(PEFR)、1秒用力呼气容积(FEV_(1))、PaO_(2)、pH水平较治疗前升高,试验组高于对照组;治疗后血清肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)、PaCO_(2)水平降低,试验组低于对照组(均P<0.05或0.001)。结论纳洛酮联合无创呼吸机治疗能改善COPD合并呼吸衰竭患者血气指标,保护肺功能,同时降低炎性因子水平,且效果显著,促进病情好转。
Objective To investigate the effects of naloxone combined with non-invasive ventilator on lung functions,PaO_(2),PaCO_(2) and pH levels in COPD patients with respiratory failure.Methods 92 subjects were selected from patients with COPD complicated with respiratory failure who were treated in Pubei People's Hospital from March 2020 to March 2021.They were divided into control group and experimental group according to random number table,with 46 cases in each group.The two groups were hospitalized for routine treatment.The control group were then given non-invasive ventilator,and the experimental group were given naloxone on the basis of the control group.Both groups were treated for 3 consecutive days.And then,clinical efficacy after treatment as well as lung functions,inflammatory factors and blood gas indexes before and after treatment were compared between the two groups.Results The total effective rate of the experimental group(93.48%)was higher than that of the control group(78.26%)after treatment.After treatment,the levels of forced vital capacity(FVC),peak expiratory flow rate(PEFR),forced expiratory volume in 1 second(FEV_(1)),PaO_(2) and pH of both groups were higher than those before treatment,and those of the experimental group were higher than those of the control group.In addition,the levels of serum tumor necrosis factor-α(TNF-α),interleukin-8(IL-8)and PaCO_(2) were decreased in the two groups after treatment,and those in the experimental group were lower than those in the control group(all P<0.05 or 0.001).Conclusion Naloxone combined with non-invasive respiratory function can improve blood gas indexes of COPD patients with respiratory failure,protect lung functions,and reduce the levels of inflammatory factors,which has significant effect,and can promote the improvement of the condition.
