摘要
目的探讨低剂量穿刺活检联合癌胚抗原(CEA)、细胞角质蛋白19片段抗原21-1(CYFRA21-1)检测对肺癌的诊断价值。方法选择150例疑似肺癌患者,所有患者均进行低剂量穿刺活检联合CEA、CYFRA21-1检测,观察患者的射线剂量、图像质量及CEA、CYFRA21-1水平。以病理诊断结果为金标准,采用Kappa一致性检验分析各检测方法的诊断结果与病理诊断结果的一致性,分析血清CEA、CYFRA21-1检测和低剂量穿刺活检单独及三者联合对肺癌的诊断价值。结果病理诊断结果显示,肺癌患者112例,肺部良性疾病患者38例。图像质量均达到进行穿刺活检的要求,且穿刺均顺利完成,成功率为100%,无穿刺失败情况。图像信号噪声比(SNR)和对比度噪声比(CNR)分别为(23.14±8.43)和(21.12±6.07)。体积CT剂量指数(CTDIvol)和剂量长度乘积(DLP)分别为(2.14±0.24)m Gy和(127.13±9.07)m Gy·cm。肺癌患者的血清CEA、CYFRA21-1水平均明显高于肺部良性疾病患者,差异均有统计学意义(P﹤0.01)。低剂量穿刺活检联合CEA、CYFRA21-1检测对肺癌的诊断结果与病理诊断结果具有很好的一致性(Kappa=0.930,P﹤0.01)。低剂量穿刺活检联合CEA、CYFRA21-1检测对肺癌的诊断效能最好,灵敏度为98.21%,特异度为94.74%,准确度为97.33%。结论低剂量穿刺活检联合CEA、CYFRA21-1检测对肺癌的诊断效能较好,优于单独检测,具有较好的临床应用价值。
Objective To explore the diagnostic value of low-dose needle biopsy combined carcinoembryonic antigen(CEA)and cyto-keratin 19 fragment antigen 21-1(CYFRA21-1)detection in lung cancer.Method A total of 150 patients with suspected lung cancer were collected.All patients underwent low-dose needle biopsy combined CEA and CYFRA21-1 detection to observe the radiographic dose,image quality and levels of CEA and CYFRA21-1.Kappa consistency test was used to analyze the consistency of each method with pathological results as the golden standard.To analyze the diagnostic value of serum CEA,CYFRA21-1 and low dose needle biopsy alone and combination in lung cancer.Result Pathological result showed that there were 112 lung cancer patients and 38 benign lung disease patients.The image quality all met the requirements of biopsy puncture,and the puncture was successfully completed with a success rate of 100%.The signal to noise ratio(SNR)and contrast-to-noise ratio(CNR)were(23.14±8.43)and(21.12±6.07),respectively.The volume CT dosimetry index(CTDIvol)and dose length product(DLP)were(2.14±0.24)m Gy and(127.13±9.07)mGy·cm,respectively.The serum CEA and CYFRA21-1 levels in lung cancer patients were higher than those in benign lung disease patients(P<0.01).Low-dose needle biopsy combined CEA and CYFRA21-1 detection showed a high consistency with pathological diagnosis(Kappa=0.930,P<0.01),and low-dose needle biopsy combined CEA and CYFRA21-1 detection showed highest diagnostic value in lung cancer,the sensitivity was 98.21%,the specificity was 94.74% and the accuracy was 97.33%.Conclusion Low dose needle biopsy combined CEA and CYFRA21-1 detection has high diagnostic efficacy in lung cancer,which is better than single detection,with good clinical application value.
