摘要
目的:观察自拟益气化痰通络汤联合西医治疗慢性阻塞性肺疾病急性加重期(AECOPD)患者的效果。方法:选取100例AECOPD患者为研究对象,采用随机数字表法分为研究组和对照组各50例。对照组行西医治疗,研究组在对照组基础上联合自拟益气化痰通络汤治疗,比较两组临床疗效、治疗前后中医证候积分、炎性因子[白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)]水平和肺功能指标[第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)及FEV_(1)占预计值的百分比(FEV_(1)%)]水平。结果:研究组治疗总有效率为96.00%(48/50),高于对照组的80.00%(40/50),差异有统计学意义(P<0.05);治疗后,研究组中医证候积分、IL-8水平和TNF-α水平均低于对照组,FEV_(1)、FVC和FEV_(1)%水平均高于对照组,差异有统计学意义(P<0.05)。结论:在西医治疗基础上采用自拟益气化痰通络汤治疗AECOPD患者可提高治疗总有效率和肺功能指标水平,降低中医证候积分和炎性因子水平,效果优于单纯西医治疗。
Objective:To observe effects of self-made Yiqi Huatan Tongluo decoction combined with western medicine in treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:100 patients with AECOPD were selected as the research objects,and were divided into study group and control group by using the random number table method,50 cases in each.The control group was treated with western medicine,while the study group was treated with self-made Yiqi Huatan Tongluo decoction on the basis of that of the control group.The clinical efficacy,the TCM syndrome score,the inflammatory factor levels[interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)],the pulmonary function index levels[forced expiratory volume in 1 second(FEV_(1)),forced vital capacity(FVC),percentage of FEV_(1) in predicted value(FEV_(1)%)]were compared between the two groups before and after the treatment.Results:The total effective rate of the study group was 96.00%(48/50),which was higher than 80.00%(40/50)of the control group,and the difference was statistically significant(P<0.05).Further,after the treatment,the TCM syndrome scores,the IL-8 and TNF-α levels in the study group were lower than those in the control group;the levels of FEV_(1),FVC and FEV_(1)% were higher than those in the control group;and the differences were statistically significant(P<0.05).Conclusions:On the basis of western medicine treatment,Yiqi Huatan Tongluo decoction in the treatment of the AECOPD patients can improve the total effective rate of treatment and the levels of pulmonary function indexes,and reduce the TCM syndrome score and the levels of inflammatory factors.Moreover,it is superior to single western medicine.
