口腔鳞状细胞癌及癌前病变患者外周血Th17/Treg平衡及细胞因子水平变化 被引量:5

Peripheral blood Th17/Treg balance and cytokine levels in patients with oral squamous cell carcinoma and precancerous lesions

摘要 目的观察口腔鳞状细胞癌(oral squamous cell carcinoma, OSCC)患者外周血Th17细胞、调节性T(regulatory T, Treg)细胞、Th17/Treg比值及细胞因子水平变化,探讨其在OSCC癌前病变进展中的意义。方法收集42例OSCC患者、47例癌前病变患者、79例体检健康者年龄、性别、体质量指数、吸烟及饮酒史等资料,进行11倾向性评分匹配。选择配对成功的32例OSCC患者(OSCC组)、32例癌前病变患者(癌前病变组)、32例体检健康者(对照组),应用流式细胞仪检测外周血Th17细胞及Treg细胞百分比,计算Th17/Treg比值;采用ELISA法检测血清白细胞介素(interleukin, IL)-2、IL-4、IL-10、转化生长因子-β_(1)(transforming growth factor-β_(1), TGF-β_(1))、干扰素-γ(interferon-γ, IFN-γ)水平;多因素logistic回归分析OSCC发生的影响因素。结果匹配前OSCC组、癌前病变组男性比率(73.81%、63.83%)、吸烟比率(71.43%、70.21%)均高于对照组(40.51%、30.38%),OSCC组吸烟、男性比率与癌前病变组比较差异均无统计学意义(P>0.05)。匹配后3组年龄,体质量指数,男性、吸烟、饮酒及合并高血压病、糖尿病、高脂血症、冠心病、慢性肾功能不全比率比较差异均无统计学意义(P>0.05)。OSCC组外周血Th17细胞百分比[(3.96±1.04)%]、Th17/Treg比值(0.76±0.17)及血清IL-4[(22.21±5.38)ng/L]、IL-10[(26.17±8.12)ng/L]、TGF-β_(1)[(56.16±10.53)ng/L]水平均高于癌前病变组[(3.06±1.24)%、0.62±0.15、(15.57±4.29)ng/L、(20.84±6.79)ng/L、(39.67±12.31)ng/L]和对照组[(1.32±0.55)%、0.57±0.11、(12.48±3.48)ng/L、(13.38±5.23)ng/L、(22.53±5.39)ng/L](P<0.05),血清IL-2[(9.08±2.33)ng/L]、IFN-γ[(8.11±1.72)ng/L]水平均低于癌前病变组[(10.72±2.15)、(8.92±2.31)ng/L]和对照组[(12.46±3.09)、(14.47±4.33)ng/L](P<0.05);癌前病变组外周血Th17细胞百分比、Th17/Treg比值及血清IL-4、IL-10、TGF-β_(1)水平均高于对照组(P<0.05),血清IL-2、IFN-γ水平均低于对照组(P<0.05);OSCC� Objective To observe the changes of peripheral blood helper Th17 cells, regulatory T cells(Treg), Th17/Treg ratio and cytokines in patients with oral squamous cell carcinoma(OSCC), and to investigate their significances in precancerous lesions. Methods The age, gender, body mass index, and smoking and drinking history were collected, and 1:1 propensity matching was performed in 42 OSCC patients(OSCC group), 47 patients with precancerous lesions(precancerous lesion group) and 79 healthy volunteers(control group). Thirty-two patients from each group were matched successfully. Flow cytometry was used to detect the percentages of Th17 cells and Treg cells, and Th17/Treg ratio was calculated. The serum interleukin(IL)-2, IL-4, IL-10, transforming growth factor-β_(1)(TGF-β_(1)) and interferon-γ(IFN-γ) were detected by ELISA. Multivariate logistic regression analysis was done to study the influencing factors of OSCC. Results Before matching, the percentages of male patients(73.81%, 63.83%) and smoking (71.43%,70.21%)in OSCC and precancerous lesion groups were higher than those in control group(40.51%,30.38%)(P<0.05),and showed no significant differences between OSCC group and precancerous lesion group(P>0.05).After matching,there were no significant differences in the age,body mass index,and percentages of patients with smoking,drinking,hypertension,diabetes,hyperlipidemia,coronary heart disease and chronic renal insufficiency history among three groups(P>0.05).Th17 cell percentage,Th17/Treg ratio,and levels of IL-4,IL-10 and TGF-β_(1) were higher in OSCC group[(3.96±1.04)%,0.76±0.17,(22.21±5.38)ng/L,(26.17±8.12)ng/L,(56.16±10.53)ng/L]than those in precancerous lesion group[(3.06±1.24)%,0.62±0.15,(15.57±4.29)ng/L,(20.84±6.79)ng/L,(39.67±12.31)ng/L]and control group[(1.32±0.55)%,0.57±0.11,(12.48±3.48)ng/L,(13.38±5.23)ng/L,(22.53±5.39)ng/L](P<0.05),while the levels of IL-2 and IFN-γwere lower in OSCC group[(9.08±2.33),(8.11±1.72)ng/L]than those in precancerous lesion group[(10.72±2.15),(8.92±2.31)
