摘要
目的观察芒针深刺会阳穴、次髎穴联合电针治疗脑卒中后便秘的临床疗效。方法将79例脑卒中后便秘患者随机分成治疗组(40例)和对照组(39例),治疗组采用芒针深刺会阳穴、次髎穴结合电针治疗,对照组采用普通针刺方法治疗。两组患者接受治疗的针刺方案均为每日1次,每周6次,持续4周,共计24次。观察两组患者治疗前后周平均自主排便次数(spontaneous bowel movements,SBMs)、Bristol粪便性状评分、便秘患者生活质量量表(patient assessment of constipation qualiy of life questionaire,PAC-QOL)评分、排便困难程度评分、临床疗效。结果治疗组临床疗效显著优于对照组(P<0.05)。与治疗前比较,两组患者治疗后周平均SBMs、Bristol粪便性状评分显著升高,PAC-QOL评分、排便困难程度评分显著降低,差异均有统计学意义(P<0.05);治疗组患者治疗后周平均SBMs、Bristol粪便性状评分升高程度和PAC-QOL评分、排便困难程度评分降低程度显著大于对照组(P<0.05)。结论芒针深刺会阳穴、次髎穴结合电针在增加脑卒中后便秘患者的自主排便次数、改善粪便性状以及排便困难程度方面均有较好的疗效。
Objective To investigate the clinical effect of deep elongated needling at Huiyang and Ciliao points combined with electroacupuncture in the treatment of post-stroke constipation.Methods A total of 79 patients with post-stroke constipation were randomly divided into treatment group with 40 patients and control group with 39 patients.The patients in the treatment group were given deep elongated needling at Huiyang and Ciliao points combined with electroacupuncture,and those in the control group were given conventional acupuncture.The acupuncture regimen was given in both groups once a day and 6 times a week for 4 consecutive weeks,with 24 times in total.The two groups were observed in terms of number of spontaneous bowel movements(SBMs),Bristol Stool Form Scale(BSFS)score,Patient Assessment of Constipation Quality of Life Questionnaire(PAC-QOL)score,and defecation difficulty score before and after treatment,and clinical outcome was also observed after treatment.Results The treatment group had a significantly better clinical outcome than the control group(P<0.05).After treatment,both groups had significant increases in mean number of SBMs and BSFS score and significant reductions in PAC-QOL score and defecation difficulty score(P<0.05),and compared with the control group after treatment,the treatment group had significantly greater increases in mean number of SBMs and BSFS score and significantly greater reductions in PAC-QOL score and defecation difficulty score(P<0.05).Conclusion Deep elongated needling at Ciliao and Huiyang points combined with electroacupuncture has a good clinical effect in patients with post-stroke constipation and can increase the number of SBMs,improve stool form,and alleviate defecation difficulty.
