摘要
目的分析良性前列腺增生(BPH)患者术后压力性尿失禁(SUI)状况的影响因素。方法前瞻性选择2019年5月—2020年11月河南信合医院收治的89例良性BPH患者,均行经尿道钬激光前列腺剜除术治疗。统计良性BPH患者术后SUI发生状况,设计患者基线资料调查表,询问患者基线资料,分析良性BPH患者术后发生SUI的影响因素。结果 89例良性BPH患者术后发生SUI 15例(16.85%);发生组合并神经系统疾病占比66.67%,较未发生组24.32%高(χ^(2)=8.449,P=0.004);发生组术前国际前列腺症状(IPSS)评分(19.20±1.26)分、术前残余尿量(76.25±4.33)mL均较未发生组(17.50±1.13)分、(68.17±3.84)mL高(t=5.212、7.274,P<0.05);经多元Logistic回归分析结果显示,合并神经系统疾病、术前IPSS评分高、术前膀胱残余尿量多是良性BPH患者术后发生SUI的危险因素(OR=10.517、3.690、1.440,P<0.05)。结论良性BPH患者术后发生SUI的影响因素可能为神经系统疾病、术前IPSS评分、术前残余尿量。
Objective To analyze the influencing factors of postoperative stress urinary incontinence(SUI) in patients with benign prostatic hyperplasia(BPH).Methods Totally 89 patients with benign BPH admitted to Henan Xinhe Hospital from May 2019 to November 2020 were prospectively selected,all patients underwent transurethral holmium laser enucleation of prostate.The occurrence of postoperative SUI in patients with benign BPH was counted,the baseline data questionnaire of patients was designed,the baseline data of patients were asked,the influencing factors of postoperative SUI in patients with benign BPH were analyzed.Results Among 89 cases of patients with benign BPH,15 cases(16.85%) had SUI;The proportion of combined neurological diseases in the occurrence group was 66.67%,higher than 24.32% in the non occurrence group(χ^(2)=8.449,P=0.004);The preoperative International Prostate Symptom Score(IPSS)(19.20±1.26) points and preoperative residual urine volume(76.25±4.33) mL were higher than those in the non occurrence group(17.50±1.13)points,(68.17±3.84)mL(t=5.212,7.274,P<0.05);By multiple Logistic regression analysis,the results showed that the risk factors of postoperative SUI in patients with benign BPH were combined neurological diseases,high preoperative IPSS score and more residual urine volume(OR=10.517,3.690,1.440,P<0.05).Conclusion The influencing factors of postoperative SUI in patients with benign BPH may be neurological diseases,preoperative IPSS score and preoperative residual urine volume.
