摘要
目的建立互联网医院安全合理用药闭环管理体系,保证患者用药安全、有效和经济。方法建立符合国家药品与邮政监管法规的药品供应目录。实施“三审三拦三沟通”的综合审方模式、基于批号追踪的药品调剂模式以及提供视频用药交代和在线用药咨询服务模式。存在质量疑问的药品可通过批号追踪程序,准确判断问题出处并给予相应处理。结果互联网医院可用药品达600余种,自2020年3月启动至2021年5月共接诊4389人次;配发药品360余种;药品出库金额127万余元。平均配送距离21.6 km,可为患者节约单程通勤时间(以公共交通计算)63.5 min,节约单程通勤费用5.9元,减少通勤期间步行距离1.53 km。处方共触发前置审核报警37次,均为2级,不合理处方干预率100%。所有药品均在最长时间48 h内送达患者指定地点,未发生药品质量投诉。药品质量可追溯率100%。线上解答患者咨询共188例次,主要问题集中于用药注意事项、药品供应与门诊及医保相关规定。结论本体系可让患者充分体验高效、便捷的安全用药管理模式,减少患者的通勤时间、费用与体力精力付出,是缓解慢病患者看病难、看病贵的有效措施。
AIM To establish a closed-loop management system for rational medication in online hospital to ensure safe,effective and economical drug use for patients.METHODS A drug supply catalog in accordance with national drug regulations and postal supervision was made.The comprehensive prescription review model of“three audits,three interceptions and three conversations”and a drug dispensing model based on“lot tracking”were implemented.A quick response code would be printed on the drug list to provide video medication explanations,and consulting service was provided online.Drugs with quality problems could be tracked through the batch number to accurately determine the problem and dealt with it accordingly.RESULTS More than 600 kinds of drugs were available in online hospital.A total of 4389 patients accepted online service and more than 360 kinds of drugs were distributed from March 2020 to May 2021.The total amount of drugs was more than 1.27 million yuan.The average delivery distance was 21.6 km,which could save patient’s one-way commuting time(in terms of public transportation)by 63.5 min,save one-way commuting cost by 5.9 yuan and reduce walking distance during commuting by 1.53 km.A total of 37 times pre-audit alarms were triggered for drug prescription,all of which were level 2,and intervention rate of unreasonable prescription was 100%.All drugs were delivered to the patient's designated place within a maximum of 48 h,and no complaints of drug quality were occurred.The traceability rate of drug quality was 100%.A total of 188 medication consultations were performed online.The main questions focused on medication precautions,drug supply,outpatient service and related regulations of medical insurance.CONCLUSION Patients can fully experience high-efficiency,convenience and safe medication,and the commuting time,cost and physical labor can also be reduced through this closed-loop management system.It is an effective measure to alleviate the difficult and expensive medical treatment of patients with chronic
