一起由鼠伤寒沙门菌污染蛋糕致食源性疾病暴发事件调查 被引量:2

Survey on an outbreak of foodborne disease caused by cake contaminated by Salmonella typhimurium

摘要 目的对一起食源性疾病暴发事件进行现场及病原分子流行病学调查,为有效防控提供科学依据。方法采用现场流行病学方法调查暴发过程,分析病例临床特征及流行病学特点;采集患者、工作人员和可疑食品(预包装未开封奶油、蛋糕、已打发奶油等)样本进行病原分离及鉴定,对分离菌株进行脉冲场凝胶电泳(PFGE)分析。结果该次暴发发病67例;潜伏期最短4 h,最长44 h,平均潜伏期(中位数)20.50 h;男性25例,女性42例,最小年龄1岁4个月,最大年龄66岁;主要症状为腹痛、腹泻、发热、呕吐和恶心,均为轻症病例,无重症和死亡病例;从15名患者采集的粪便标本中检出12株鼠伤寒沙门菌,10名工作人员肛拭子标本中检出1株鼠伤寒沙门菌,1批次蛋糕胚、1批次预包装未开封超高温灭菌搅打稀奶油、4批次生日蛋糕(含1份病患未食用完蛋糕)、2批次已打发的奶油中均检出鼠伤寒沙门菌;21株鼠伤寒沙门菌经PFGE分析,同源性为100.00%。结论综合流行病学调查、实验室病原学检测及分子分型,证实本起事件为同一来源的鼠伤寒沙门菌引起的食源性疾病暴发。 Objective To conduct the field and molecular epidemiological investigation of a foodborne disease outbreak,so as to provide a scientific basis for effective prevention and control.Methods Field epidemiological methods were carried out to investigate the outbreak process to analyze the clinical and epidemiological characteristics of the cases.Samples of patients,staff and suspicious foods(pre-packed unopened cream,cake,whipped cream,etc.)were collected for pathogen isolation and identification.The isolates were analyzed by pulsed field gel electrophoresis(PFGE).Results A total of 67 cases were included in the outbreak.The shortest incubation period was 4 h,the longest was 44 h,and the mean incubation period(median)was 20.50 h.There were 25 males and 42 females,with a minimum age of 1 year and 4 months and a maximum age of 66 years.The main symptoms were abdominal pain,diarrhea,fever,vomiting and nausea,all of which were mild cases,and no severe cases or death cases were found.The stool specimens which were collected from 15 patients detected 12 strains of Salmonella typhimurium.The anal swab specimens which were collected from 10 staff members detected 1 strains of Salmonella typhimurium.Salmonella typhimurium were detected in 1 batch of cake embryo,1 batch of prepackaged unsealed ultra-high temperature sterilized whipped cream,4 batches of birthday cake(including 1 piece of cake not eaten by the patient)and 2 batches of whipped cream.PFGE analysis of 21 strains of Salmonella typhimurium showed a homology of 100.00%.Conclusion The results of epidemiological investigation,laboratory etiological detection and molecular typing comprehensively confirmed that the event is a foodborne disease outbreak caused by Salmonella typhimurium of the same origin.
