摘要
目的探讨不同时间点行术前经导管动脉栓塞术(TAE)对ShamblinⅡ/Ⅲ型颈动脉体瘤(CBTs)外科切除术的影响。方法回顾性分析2010年1月至2020年1月本院收治的ShamblinⅡ/Ⅲ型CBTs患者的资料,根据纳入排除标准及不同时间间隔将CBTs分为两组:≤24 h组及>24 h组,并根据不同时间间隔行1∶1倾向性匹配(PSM)。分析并比较PSM后两组患者临床资料、手术方式、术中出血量、手术时间、并发症等相关指标。结果本研究共纳入79例患者82个病灶(≤24 h组、>24 h组各41个病灶)。≤24 h组与>24 h组肿瘤大小分别为42.7 mm、42.1 mm(P=0.8)。术中出血量以及手术时间分别为131.2 ml、292.6 mL(P=0.03),169.5 min、253.0 min(P=0.002)。≤24 h组与>24 h组术中血管重建率分别为19.5%和34.2%(P=0.09)、并发症的发生率分别为26.8%和29.3%(P=0.8)。结论 TAE术后24 h内行外科手术切除CBTs可有效降低术中失血量,改善手术视野可视化,从而有利于ShamblinⅡ/Ⅲ级CBTs的手术切除,减少手术时间。
Objective To explore the effect of the optimal time interval between preoperative transarterial embolization(p-TAE) and surgeryof carotid body tumors(CBTs) by analyzing surgery-related indicators. Methods This retrospective study reviewed 82 tumors in79 patients in the First Affiliated Hospital, Sun Yat-sen University between Jan 2010 and Jan 2020,which underwent surgical resection for CBTs.According to interval time between TAE and surgery, the CBTs were classified into two groups: ≤ 24 hours group and > 24 hours group.Demographics, periprocedural details, and postoperative outcomes were analyzed. Results A total of 82 CBTs were excised in 79 patients.The average tumor size was 42.7 vs.42.1 mm(P= 0.8) for ≤ 24 hours group and > 24 hours group.The mean operative time(169.5 vs.253.0 mins, P= 0.002) and intraoperative blood loss(131.2 vs.292.6 mL,P= 0.03) were less in the ≤ 24 hours group.The incidence of revascularization required(19.5%and 34.2%,P= 0.09) and total complications(26.8% and 29.3%,P= 0.8) were lower in the ≤ 24 hours groupwhen compared to > 24 hours group. Conclusion Our results indicate that within 24-hour procedure is superior to over 24-hour procedurebetween preoperative embolization of feeding arteries and resection of CBTsin terms of intraoperative blood loss and operative time.
