摘要
目的了解产后抑郁症患者的成人依恋及领悟社会支持状况,并探讨抑郁症状与成人依恋、领悟社会支持的关系。方法对82例产后抑郁症患者(研究组)和76名健康产妇(对照组)采用亲密关系经历量表、领悟社会支持量表评定成人依恋及领悟社会支持状况,采用汉密顿抑郁量表评定产后抑郁症患者的抑郁症状。比较两组成人依恋及领悟社会支持状况,采用Pearson相关分析、多元回归分析探讨抑郁症状与成人依恋、领悟社会支持的关系,并分析领悟社会支持在成人依恋和抑郁症状之间的中介效应。结果研究组亲密关系经历量表的依恋焦虑、依恋回避因子分均显著高于对照组(P<0.05),领悟社会支持量表的家庭内支持、家庭外支持因子分均显著低于对照组(P<0.01)。产后抑郁症患者汉密顿抑郁量表总分与亲密关系经历量表的依恋焦虑、依恋回避因子分呈显著正相关(P<0.01),与领悟社会支持量表的家庭内支持、家庭外支持因子分呈显著负相关(P<0.01),依恋焦虑因子分对汉密顿抑郁量表总分有显著预测作用(P<0.01)。领悟社会支持在成人依恋与产后抑郁之间起部分中介效应(P<0.05),其中介效应占总效应的57.4%。结论产后抑郁症患者成人依恋及领悟社会支持状况较差,成人依恋是产后抑郁的预测因子,领悟社会支持在产后抑郁与成人依恋之间起部分中介效应。
Objective To understand the status of adult attachment and perceived social support in patients with postpartum depression,and to explore the relationship between depressive symptoms and adult attachment and perceived social support.Methods Eighty-two patients with postpartum depression(study group)and 76 healthy puerperae(control group)were evaluated by experiences in close relationship inventory(ECR)and perceived social support scale(PSSS)to assess the status of adult attachment and perceived social support.The depressive symptoms of patients with postpartum depression were evaluated by Hamilton depression scale(HAMD).The status of adult attachment and perceived social support in the two groups were compared.Pearson correlation analysis and multiple regression analysis were used to explore the relationship between postpartum depression and adult attachment and perceived social support,and to analyze the mediating effect of perceived social support between adult attachment and depressive symptoms.Results The attachment anxiety and attachment avoidance factor scores of ECR in the study group were significantly higher than those in the control group(P<0.05),and the intra-family support and extra-family support factor scores in PSSS were significantly lower than those in the control group(P<0.01).The total score of HAMD in patients with postpartum depression was significantly positively correlated with the attachment anxiety and attachment avoidance factor scores of ECR(P<0.01),and was significantly negatively correlated with the intra-family support and extra-family support factor scores of PSSS(P<0.01).The attachment anxiety factor score had a significant predictive effect on the total score of HAMD(P<0.01).Perceived social support had a partial mediating effect between adult attachment and postpartum depression(P<0.05),and the mediating effect accounted for 57.4%of the total effect.Conclusions Adult attachment and perceived social support in patients with postpartum depression are poor.Adult attachment is a pred
