摘要
目的 通过meta分析的方法确定中国痴呆患者肺部感染的相关危险因素。方法 检索中国期刊全文数据库(CNKI)、中国生物医学文献服务系统(CBM)、维普数据库(VIP)、万方、PubMed、Web of Science、Embase及Cochrane Library等数据库,收集中国痴呆患者肺部感染危险因素的相关研究,检索年限为2000年1月—2021年3月,由2位研究者筛选文献、提取资料,并进行质量评价。利用RevMan 5.3软件进行meta分析。结果 共纳入22个研究,16个相关危险因素,4 869例患者,其中肺部感染组1 370例,非感染组3 499例。年龄≥65岁、吸烟、卧床、住院时间≥30天、应用抗菌药物、抗精神病药物、侵入性操作、吞咽困难、糖尿病、低蛋白血症及肿瘤是导致痴呆患者肺部感染的危险因素(均P<0.05)。结论 年龄大、吸烟、长期卧床、住院时间长、应用抗菌药物、应用抗精神病药物、侵入性操作、吞咽困难、糖尿病、低蛋白血症及肿瘤是中国痴呆患者肺部感染的危险因素。
Objective To determine the risk factors of pulmonary infection in Chinese dementia patients by meta-analysis. Methods CBM,CNKI,VIP,Wanfang,PubMed,Web of Science,Embase and Cochrane Library and other databases were searched to collect relevant studies on risk factors of lung infection in Chinese dementia patients. The search period is from January 2000 to March 2021.Two researchers screened the literature,extracted the data,and evaluated the quality. Meta-analysis was carried out using RevMan Totally 5.3. Results Totally 22 studies,16 related risk factors and 4 869 patients were included,and there were 1 370 cases in the lung infection group and 3 499 cases in the non-infection group. Age ≥65 years old,smoking,bed rest,hospital stay ≥30 days,use of antibacterial drugs,use of antipsychotic drugs,invasive procedures,dysphagia,diabetes,hypoproteinemia,and tumor were the risk factors for lung infection in patients with dementia(all P<0.05). Conclusion Old age,smoking,long-term bed rest,long hospital stay,use of antibiotics,use of antipsychotics,invasive procedures,dysphagia,diabetes,hypoproteinemia,tumors are risk factors for lung infections in Chinese patients with dementia.
