Meta-analysis of HPV infection in esophageal cancer patients

摘要 目的通过Meta分析综合评估食管癌(Esophageal Cancer,EC)与人乳头瘤病毒(Human papillomavirus,HPV)感染的相关性。方法以食管癌和HPV为检索词,检索PubMed、Web of Science两大数据库中有全文的英文文献,应用软件Stata SE 12.0对所收集数据进行统计分析,探讨食管癌发生与HPV感染的病因学关联。结果根据筛选标准共纳入11篇文献,共有食管癌患者1169例,对照组1382例;根据异质性检验结果采用随机效应模型进行Meta分析,结果显示:世界范围内食管癌患者平均HPV阳性率为36.10%,OR:4.20,95%Cl:2.38~7.43,Z检验结果显示z值为4.94,P<0.05,表明食管癌发生与HPV感染具有相关性。亚组分析结果显示,食管癌患者地域分布与HPV感染无明显关联,而病例所在地区食管癌发病率高低、患者性别以及淋巴结转移情况与HPV感染有关,且女性食管癌患者发病率、HPV感染阳性率均低于男性患者(37.54%、46.26%:62.46%、50.28%);发表偏倚估计及敏感性分析结果显示文献不存在发表偏倚,结果较为可靠。结论通过对世界范围内食管癌患者HPV感染状况进行Meta分析,发现食管癌病例所在地区发病率高低、患者性别以及淋巴结转移情况与HPV感染相关。 Objective To comprehensively evaluate the correlation between esophageal cancer and human papillomavirus infection by Meta-analysis.Methods The full text of English papers in PubMed and Web of Science were searched using esophageal cancer and HPV as the keywords,and the collected data were analyzed to investigate the etiological relationship between esophageal cancer and HPV infection through software Stata SE 12.0.Results Total of 11 literatures including 1169 esophageal cancer patients and 1382 normal controls were selected according to the screening criteria.The random-effect model was used for meta-analysis according to the heterogeneity test results,and the result showed that the average HPV positive rate of esophageal cancer patients was 36.10%,OR:4.20(95%Cl:2.38-7.43)in worldwide.Z test result showed that z value was 4.94,P<0.05,which indicated that the occurrence of esophageal cancer was correlated with HPV infection.The results of subgroup showed that the regional distribution of esophageal cancer patients was not significantly associated with HPV infection,but the incidence of esophageal cancer cases in the region,gender and lymph node metastasis of the esophageal cancer patients were associated with HPV infection;the incidence rate and HPV infection rate of female esophageal cancer patients were lower(37.54%,46.26%)than that of male patients(62.46%,50.28%).The results of publication bias estimation and sensitivity analysis showed that there was no publication bias and the results were reliable.Conclusion The incidence of esophageal cancer cases in the region,gender and lymph node metastasis are related to HPV infection through the meta-analysis of HPV infection in esophageal cancer patients in worldwide.
