摘要
目的:比较聚氯乙烯和泡沫骨骼骨折教学模型在手术技能培训中的应用效果。方法:将2018年1月至2019年6月进行骨科规范化培训的60名学生作为研究对象,所有学生均接受骨折复位内固定的理论知识培训和观看骨折复位固定的手术操作视频。按照随机数字表法分为两组各30名学生。聚氯乙烯(PVC)组学生在PVC塑料管上模拟骨折复位固定的手术操作,模型组学生在泡沫骨骼模型上模拟骨折复位固定的手术操作。骨科规范化培训结束时对两组学生进行理论考核和满意度评价,并记录在标准SYNBONE骨折模型上进行复位固定的效果和操作时间。结果:骨科规范化培训结束时,两组学生的理论考核成绩比较无统计学差异,模型组学生技能考核成绩较PVC组高,且差异有统计学意义(P<0.05)。两组学生在胫骨骨折模型上进行复位固定的优良率比较无统计学差异。虽然模型组操作时间快于PVC组,但差异无统计学意义。模型组学生的满意度高于PVC组,但差异无统计学意义。结论:聚氯乙烯塑料管和泡沫骨骼模型均可实现骨折复位固定的手术模拟教学。前者材料成本低、方便获取,适合基层教学推广;后者符合骨骼解剖形态,可提高学生培训满意度,但价格昂贵。
Objective: To compare the application effect of two different fracture teaching models in surgical skill training. Methods: A total of 60 students were enrolled in the orthopedics department of our hospital from January 2018 to June 2019. All students received theoretical knowledge training of fracture reduction and internal fixation and watched surgical operation videos of fracture reduction and fixation. According to the random number table method, the students were divided into two groups, with 30 participants in each group.Students in PVC group simulated the surgical operation of fracture reduction and fixation on a PVC plastic tube, and students in model group simulated the surgical operation of fracture reduction and fixation on a foam bone model. At the end of the orthopedic training, the two groups of students were assessed on theory, skills and questionnaire adjustment. The effect of reduction and fixation on a standard SYNBONE fracture model and the duration of operation were recorded. Results: At the end of regulation training, there was no statistical difference between the two groups of students in theoretical assessment. The score of skill examination of model group was higher than that of PVC group(P<0.05). The excellent and good rates of reduction and fixation in the tibial fracture model were compared, although the operation time of model group was faster than that of PVC group, there was no statistical difference between the two groups. The satisfaction of model group was higher than that of PVC group, but the difference was not statistically significant. Conclusions: Both PVC tube and foam bone model can be used to simulate the teaching of fracture reduction and fixation. The former material cost is low with convenient access, suitable for grassroots teaching promotion;the latter conforms to the skeletal anatomy and can improve students’ training satisfaction, but the price is expensive.
