摘要
目的了解某三甲医院冠脉支架患者疾病不确定感现状,探究其与日常生活能力的关系。方法采取整群抽样法,选取西安某三甲医院心内科2020年5—12月内符合纳入标准的187例冠脉支架术后住院患者作为研究对象。采用一般资料调查表、中文版Mishel疾病不确定感量表(MUAS-A)及日常生活能力指数量表(BI)进行研究。结果冠脉支架术后住院患者疾病不确定感得分为(71.89±11.99)分,处于中等水平,其中不明确性维度得分最高;自理能力得分为(96.50±8.22)分,大部分患者(67.91%)能够完全自理;日常生活能力与疾病不确定感呈负相关(r=-0.192,P<0.05),其中主要是疾病不确定感的不明确性维度与日常生活能力呈负相关(r=-0.185,P<0.05)。多元逐步线性回归分析显示,患者的自评经济压力、受教育程度、对疾病了解程度是影响疾病不确定感的重要因素。结论提高冠脉支架患者自理能力和给予有效的信息支撑对于减轻冠脉支架患者疾病不确定感、提高其生活质量有重要意义。
Objective To explore the current status of disease uncertainty in patients after coronary stenting in a third grade hospital and analyze the correlation between uncertainty in illness and ability of daily living.Methods The cluster sampling method was adopted to select patients who met the criteria and were hospitalized after coronary stenting in the Department of Cardiology of a grade A Hospital in Xi’an from May to December 2020 as the researching object.A total of 187 patients were evaluated by using general information questionnaire,Mishel uncertainty in illness scale for adults(MUIS-A)and Barthel index(BI).Results The total score of illness uncertainty in patients after coronary stenting was(71.89±11.99)points,which was in a moderate level.Among them,the undefined dimension scored the highest.Self-care score was(96.50±8.22)points,most patients(67.91%)were able to take care of themselves.Illness uncertainty were negatively correlated with activity of daily living(r=-0.192,P<0.05),in which the uncertainty dimension of disease uncertainty was negatively correlated with activity of daily living(r=-0.185,P<0.05).The analysis results of multiple linear stepwise regression model showed that self-rated economic pressure,education level and knowledge of disease were the factors affecting disease uncertainty.Conclusion Improving the self-care ability of patients after coronary stenting and providing effective information support are of great significance to reduce the disease uncertainty and improve the quality of patients after coronary stenting.
