摘要
目的比较腰-骶手法调整与腰椎斜扳法、骶髂关节手法调整治疗腰椎间盘突出症(LDH)合并骶髂关节紊乱的临床疗效。方法将90例腰椎间盘突出症伴骶髂关节紊乱患者随机分为腰椎斜扳组、骶髂关节调整组、腰-骶手法调整组各30例,先用常规推拿手法放松腰骶部肌肉,再分别用腰椎斜扳法、骶髂关节调整手法、骶髂关节调整结合腰椎斜扳法;均隔日治疗1次,疗程为20 d。各组治疗前后用疼痛视觉模拟(VAS)评分、日本骨科学会下腰痛评定量表(JOA)评价临床疗效。结果腰椎斜扳组、骶髂关节调整组有效率分别为76.67%、73.33%,低于腰-骶手法调整组的96.67%(P<0.05)。各组治疗后VAS评分均降低(P<0.05),腰-骶手法调整组VAS评分低于腰椎斜扳组、骶髂关节调整组(P<0.05)。治疗后腰-骶手法调整组与腰椎斜扳组JOA评分均增加(P<0.05),且腰-骶手法调整组JOA评分高于腰椎斜扳组、骶髂关节调整组(P<0.05)。结论骶髂关节手法调整联合腰椎斜扳法能有明显改善腰椎间盘突出症伴骶髂关节紊乱的临床症状,疗效优于单纯地使用骶髂关节调整手法或腰椎斜扳法。
Objective:To compare the clinical efficacy of lumbo-sacral manipulation adjustment with lumbar oblique pulling and sacroiliac joint manipulation in the treatment of lumbar disc herniation(LDH)combined with sacroiliac joint disorders.Methods:A total of 90 patients with lumbar disc herniation and sacroiliac joint disorder were randomly divided into the sacroiliac joint manipulation adjustment group,lumbar oblique pulling group,and lumbo-sacral manipulation adjustment group(30 cases each).The conventional massage was used to relax the lumbosacral muscles,and then lumbar obliquity manipulation,sacroiliac joint adjustment manipulation,the combination of the two methods were used.All patients were treated once every other day for 20 days.Visual analogue scale(VAS)and Japanese Society of Osteology Low back Pain Rating Scale(JOA)before and after treatment were recorded to evaluate the clinical efficacy.Results:The effective rates were 76.67%and 73.33%in the lumbar obturate manipulation group and the sacroiliac joint adjustment group,respectively,and 96.67%in the lumbosacral manipulation group(P<0.05).The VAS scores in all three groups decreased after treatment(P<0.05);the VAS score of the lumbo-sacral manipulation adjustment group was significantly lower than those in the other two groups(P<0.05).After treatment,the JOA score increased in both the lumbosacral manipulation adjustment group and the lumbar obscenity manipulation group(P<0.05).The increase of JOA score in the lumbo-sacral manipulation adjustment group was statistically different from that in the lumbar obscenity manipulation group and the sacroiliac joint adjustment group(P<0.05).Conclusion:Sacroiliac joint manipulation combined with lumbar obliquity manipulation can significantly improve the clinical symptoms of lumbar disc herniation with sacroiliac joint disorder,and the efficacy is better than using sacroiliac joint manipulation or lumbar obliquity manipulation alone.
