摘要
目的描述肺部超声在新型冠状病毒肺炎(简称新冠肺炎)的影像学征象,探讨肺部超声在新冠肺炎病情评估方面的应用价值。方法选取玉林市新冠肺炎病例为研究对象,以肺部高分辨CT(High-resolution CT,HRCT)为金标准,同步行肺部超声检查,记录CT定量参数和肺部超声的数据,分析肺部超声B线评分(LUBS)与肺部病灶总体积、肺部病灶密度平均CT值和受累肺段数量的关联性,并建立数据模型。配对分析治疗前后LUBS和肺部病灶总体积变化的差异性和关联性,并评价肺部超声对胸膜线形态和肺实变的诊断效能。结果肺部超声在新冠肺炎诊疗期间探及数量不等的B线、胸膜线异常20例次、肺实变11例次。LUBS与肺部病灶总体积、肺部病灶密度平均CT值、受累肺段数量均呈正相关关系(P<0.05),分别建立最优的回归方程。治疗前后新冠肺炎肺部病灶总体积和LUBS的变化均有统计学意义(P<0.05),而治疗前后肺部病灶总体积和LUBS的变化呈非线性相关(r=0.569,P>0.05)。肺部超声在胸膜线形态和肺实变方面的诊断效能均无统计学差异(P>0.05),诊断胸膜线形态和肺实变的灵敏度、特异度和符合率较高。结论肺部超声在床旁能准确反映新冠肺炎病灶的影像信息,有助于实时、安全而便捷地评估诊疗效果,在新冠肺炎病情评估方面可作为CT检查的有力补充手段。
Objective To describe the imaging features of lung ultrasound in COVID-19(COVID-19 for short),and explore the application value of lung ultrasound in the evaluation of COVID-19’s condition.Methods Confirmed COVID-19 cases in Yulin city were selected as the study subjects,and the lung ultrasound examinations were performed simultaneously with high-resolution CT as the gold standard.The data of CT quantitative parameters and lung ultrasound was recorded.The correlations between lung ultrasound B-line score(LUBS)and total volume of lung lesions,average CT value of lung lesion density and number of affected lung segments were analyzed,and the data models were established.The difference and correlation between LUBS and the total volume of lung lesions before and after treatment were analyzed,and the diagnostic efficiency of lung ultrasound in pleural line shape and lung consolidation was evaluated.Results During the diagnosis and treatment of COVID-19,lung ultrasound detected a number of B-line,20 cases of pleural line abnormality and 11 cases of lung consolidation.LUBS was positively correlated with the total volume of lung lesions,the average CT value of lung lesion density,and the number of affected lung segments(P<0.05),and the optimal regression equations were established respectively.The changes of total volume of lung lesions and LUBS before and after treatment were statistically significant(P<0.05),while the changes of total volume of lung lesions and LUBS before and after treatment were non-linear(r=0.569,P>0.05).There was no statistical difference in the diagnostic efficacy of lung ultrasound on pleural line morphology and lung consolidation(P>0.05),and the sensitivity,specificity and coincidence rate in the diagnosis of pleural line shape and lung consolidation were higher.Conclusion Lung ultrasound could accurately reflect the imaging information of COVID-19’s lesions at the bedside,which was helpful to evaluate the effect of diagnosis and treatment in real-time,safe and convenient,and could be used as
