摘要
目的探讨局部麻醉下外套式剥脱导管联合点状剥脱术治疗原发性大隐静脉曲张的效果。方法选取2019年1月至2021年1月山东大学第二医院收治的482例原发性大隐静脉曲张患者,按照随机数字表法将其随机分为观察组(n=220)和对照组(n=262),观察组患者采用局部麻醉下外套式剥脱导管联合点状剥脱术,对照组患者采用神经阻滞麻醉、硬膜外麻醉、全身麻醉下高位结扎剥脱术。观察两组患者的麻醉用时、手术时间、术中出血量、术后住院时间、术后首次下床时间、围手术期禁饮食时间,比较两组患者的术后并发症发生情况。结果两组患者麻醉用时、手术时间、术后首次下床时间、围手术期禁饮食时间、术后住院时间均明显短于对照组,术中出血量明显少于对照组,差异均有统计学意义(P<0.01)。两组患者术后并发症发生率比较,差异无统计学意义(P>0.05)。结论局部麻醉下外套式剥脱导管联合点状剥脱术治疗原发性大隐静脉曲张,具有创伤小、恢复快、住院时间短等优势,值得临床推广应用。
Objective To investigate the effect of outer sleeve stripping catheter combined with punctate stripping in the treatment of primary saphenous varices under local anesthesia.Method A total of 482 patients with primary saphenous varicose vein admitted to the Second Hospital of Shandong University from January 2019 to January 2021 were selected and randomly divided into the observation group(n=220)and the control group(n=262)according to the random number table method.The patients in the observation group were given the outer sleeve stripping catheter combined with punctate stripping under local anesthesia,and the patients in the control group were treated with nerve block anesthesia,epidural anesthesia,high ligation and stripping under general anesthesia.The anesthesia time,operation time,intraoperative blood loss,postoperative hospital stay,postoperative first ambulation time,and perioperative fasting time were observed in the two groups of patients,and the incidence of complications in the two groups was also compared.Result The anesthesia time,operation time,postoperative first out of bed time,perioperative fasting time,and postoperative hospital stay in the two groups were significantly shorter than those in the control group,and the intraoperative blood loss volume was significantly less than that in the control group,and the differences were statistically significant(P<0.01).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion Outer sleeve stripping catheter combined with punctiform stripping in the treatment of primary saphenous varicose veins under local anesthesia has the advantages of limited trauma,fast recovery,and short hospital stay,which is worthy of clinical application.
