摘要
目的探讨核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)及微小RNA-22-3p(miR-22-3p)在儿童结肠息肉组织中的表达及对儿童结肠息肉疾病进展的影响。方法选取结肠息肉患儿98例,其中幼年性息肉患儿52例、腺瘤性息肉患儿46例,另选取相对应患儿正常结肠组织作为对照;采用实时荧光定量PCR(qRT-PCR)法检测miR-22-3p相对表达量,免疫组化法检测NLRP3表达,卡方检验分析不同临床病理特征结肠息肉组织中NLRP3及miR-22-3p表达;单因素及多因素logistic回归分析影响儿童结肠息肉疾病进展的因素。结果正常结肠组织、幼年性息肉组织、腺瘤性息肉组织中NLRP3阳性表达率依次升高,miR-22-3p相对表达量依次降低,各组间比较差异有统计学意义(P<0.05);腺瘤性息肉、息肉多发、息肉直径≥2.5 cm的结肠息肉患儿中NLRP3阳性表达、miR-22-3p低表达的比例升高(P<0.05);NLRP3是影响结肠息肉发展为腺瘤性的独立危险因素(P<0.05),miR-22-3p是影响结肠息肉发展为腺瘤性的保护因素(P<0.05)。结论结肠息肉组织中NLRP3高表达、miR-22-3p低表达,且二者与结肠息肉疾病进展有关。
Objective To investigate the expression level changes of nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)and microRNA-22-3p(miR-22-3p)of children with colon polyps(CP)and the influencing factors of the progression of children with CP,so as to provide clinical indicators for the treatment of CP.Methods Ninety-eight children with CP were selected as the research objects,including 52 cases with juvenile polyps and 46 cases with adenomatous polyps;normal colon tissues of the corresponding children were selected as controls.Real-time fluorescent quantitative PCR(qRT-PCR)method was used to detect the relative expression of miR-22-3p,immunohistochemical method was used to detect the expression of NLRP3;Chi square test was used to analyze the expression difference of NLRP3 and miR-22-3p in CP with different clinicopathological features;single factor and multivariate logistic regression were used to analyze the factors affecting the progression of children's CP.Results The positive expression rate of NLRP3 in normal colon tissues,juvenile polyp tissues,and adenomatous polyp tissues increased successively,and the relative expression of miR-22-3p decreased successively,and the difference between the two groups was statistically significant(P<0.05).The positive expression of NLRP3 and low expression of miR-22-3p were higher in children with adenomatous polyps,multiple polyps and polyp diameter≥2.5 cm(P<0.05).The NLRP3 and miR-22-3p in CP were negatively correlated(r=-0.419,P<0.05).NLRP3 was an independent risk factor for CP to develop adenomatous polyps(P<0.05),miR-22-3p was a protective factor for CP to develop adenomatous polyps(P<0.05).Conclusion NLRP3 is highly expressed in CP tissues,and miR-22-3p is lowly expressed,and both of them are related to the progression of CP,which may be of great significance in monitoring the progression of CP.
