摘要
目的探讨胎儿前髓帆间隙囊性病变产前诊断及分析其生后对生长发育的影响。方法选择2016年1月至2020年1月于广西柳州市人民医院接受常规超声检查的26976名孕妇产前诊断发现前髓帆间隙囊性病变的27例胎儿为研究对象。分析前髓帆间隙囊性病变声像图特征,定期随访复查,评估其神经系统发育及体格生长发育。结果超声声像图表现:在横切面及矢状面均可观察到颅内囊性病变位于脑中线处丘脑后方、胼胝体压部前下方、第三脑室顶部上方、穹隆联合的下方及大脑内静脉上方;27例囊性病变均未合并脑室扩张或其他结构异常,平均直径为12(9~17)mm,其中16例囊性病变大小在整个妊娠期间保持稳定,7例在妊娠后期较初次略有增大,4例产前末次检查消失;新生儿随访观察时间为8~48个月,新生儿超声及核磁检查证实27例胎儿均存在前髓帆间隙囊性病变,其中2例在生后1个月消失,其余在随访期间囊性病变大小保持稳定,神经系统及体格生长发育表现未见异常。结论本组产前确诊为孤立性前髓帆间隙囊性病变胎儿出生后预后良好。超声检查简便、快速,能准确地判断胎儿颅内解剖结构,是产前诊断胎儿前髓帆间隙囊性病变的首选方法。
Objective To investigate the prenatal diagnosis of fetal premedullary sail gap cystic lesions and analysis its effect on growth and development after birth.Methods Among the 26976 pregnant women who underwent routine ultrasound at Liuzhou People′s Hospital from January 2016 to January 2020,27 fetuses with prenatal diagnosis of premedullary sail gap cystic lesions were selected as the research objects.The acoustic image characteristics of premedullary sail gap cystic lesions were analyzed,and regular follow-up examinations were conducted to evaluate the development of nervous system and physical growth.Results Ultrasonographic findings:In both cross section and sagittal plane,intracranial cystic lesions were observed posterior to the thalamus at the cerebral midline,before and below the pressure portion of the corpus callosum,above the top of the third ventricle,below the union of the dome,and above the inner brain vein.None of the 27 cases of cystic lesions combined with ventricular dilatation or other structural abnormalities.The average diameter was 12(9-17)mm.Among them,the size of cystic lesion remained stable throughout pregnancy in 16 cases,the size of cystic lesion increased slightly in late pregnancy compared with the first time in 7 cases,the cystic lesion disappeared at the final antenatal examination in 4 cases.Neonatal follow-up observation time was 8-48 months.Neonatal ultrasound and magnetic resonance imaging confirmed that all there was premedullary sail gap cystic lesion in all 27 cases,of which 2 cases disappeared about 1 month after birth,and the cystic lesion size of the rest remained stable during the follow-up period,no abnormalities occurred in the nervous system and physical growth and development.Conclusion The prenatal diagnosis of isolated premedullary sail gap cystic lesion in this group has a good prognosis after birth.Ultrasound examination is simple and rapid,and can accurately judge fetal intracranial anatomical structure.It is the first choice for prenatal diagnosis of fetal prem
