基于营养风险评估的个性化饮食指导在腹膜透析患者中的应用 被引量:3

摘要 目的:探讨营养风险评估的个性饮食指导对腹膜透析患者饮食管理行为及生活质量的影响。方法:选取某院接受腹膜透析治疗的患者116例,采用随机数字表法分为干预组和对照组,每组58例。对照组给予常规营养护理干预,干预组在营养风险评估的基础上实施个性化饮食指导。随访3个月,比较2组患者饮食管理行为、营养状况[总蛋白(TP)、前白蛋白(PA)、白蛋白(ALB)、Hb]及生活质量[肾脏疾病及透析相关生存质量评分(KDTA)、简明健康调查表(SF-36)评分]。结果:观察组患者饮食管理认知水平、饮食管理行为能力、饮食管理自我效能评分均明显高于对照组,差异有统计学意义(P<0.05);观察组TP、PA、ALB、Hb营养指标均优于对照组(P<0.05);干预后观察组KDTA、SF-36评分高于对照组,差异有统计学意义(P<0.05)。结论:营养风险评估的个性化饮食指导有助于促进腹膜透析患者饮食管理行为能力的养成,改善患者的营养状况及生活质量。 Objective:To investigate the effects of personalized dietary guidance based on nutritional risk assessment on dietary management behavior and quality of life in peritoneal dialysis patients.Methods:116 patients who underwent peritoneal dialysis in a hospital were enrolled and randomly divided into an intervention group and a control group.The control group received routine nutritional nursing,while the intervention group was given the personalized dietary guidance based on nutritional risk assessment.After 3 months of follow-up,the dietary management behavior,nutritional status,quality of life and other indicators were compared between the two groups.Results:After the intervention,the intervention group's diet management cognition level,diet management behavior ability,and diet management self-efficacy score were significantly higher than those of the control group,the differences being statistically significant(P<0.05).The nutritional indexes such as serum total protein(TP),prealbumin(PA),serum albumin(ALB)and hemoglobin(HB)were significantly higher than those of the control group,the differences being statistically significant(P<0.05).Kidney disease and dialysis-related quality of life(KDTA)score and concise health questionnaire(SF-36)score of the intervention group were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion:Personalized dietary guidance based on nutritional risk assessment contributes to the development of dietary management behavior in patients with peritoneal dialysis and improvement of their nutritional status and quality of life.
