摘要
目的探讨冠心宁方联用常规西药对系统性红斑狼疮(SLE)患者左心功能的影响,评价其用于SLE合并心血管损害中的临床疗效及安全性。方法选取浙江中医药大学附属第二医院风湿免疫科2019年1月至2020年6月的SLE合并心血管损害的患者56例,随机分为两组,对照组、治疗组各28例,对照组给予泼尼松片及硫酸羟氯喹片治疗,治疗组在对照组治疗措施基础上加用冠心宁方煎剂,经6周治疗,对比两组治疗效果、心功能指标及症状体征改善情况以及不良反应发生情况。结果经治疗,治疗组总有效率为89.28%(25/28),明显高于对照组总有效率71.43%(20/28),差异有统计学意义(P<0.01),且两组均未发生与药物治疗相关的不良反应;两组各项心功能指标水平、心脏症状体征、SLEDAI评分较给药前均有所改善,且治疗组改善效果更加显著(P<0.05)。结论冠心宁方联用常规西药可改善SLE合并心血管损害患者的症状及体征,改善左心功能,具有一定的临床应用价值。
Objective To investigate the effect of Guanxinning prescription combined with conventional western medicine on left ventricular function in patients with SLE,and to evaluate its clinical efficacy and safety in SLE with cardiovascular damage.Methods A total of 56 SLE patients with cardiovascular damage were selected from the Department of Rheumatology and Immunology of the Second Affiliated Hospital of Zhejiang Chinese Medicine University from January 2019 to June 2020 and randomly divided into two groups,with 28 patients in the control group and 28 patients in the treatment group.The control group was treated with prednisone tablets and hydroxychloroquine sulfate tablets,and the treatment group was treated with Guanxinning Decoction on the basis of the treatment measures of the control group.After 6 weeks of treatment,the therapeutic effect,improvement of cardiac function indexes,symptoms and signs,and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the total effective rate of the treatment group was 89.28%(25/28),which was significantly higher than 71.43%(20/28)of the control group,and the difference was statistically significant(P<0.01).No adverse reactions related to drug therapy were observed in the two groups.The levels of cardiac function indexes,cardiac symptoms and signs,and the SLEDAI score in both groups were improved compared with those before administration,and the improvement effect in the treatment group was more significant(P<0.05).Conclusion Guanxinning prescription combined with conventional western medicine can improve the symptoms and signs of SLE patients with cardiovascular damage,improve left ventricular function,and has certain clinical application value.
