2018年青岛部分地区儿童呼吸道感染病原体分析 被引量:4

AN ANALYSIS OF PATHOGENS CAUSING RESPIRATORY INFECTION IN CHILDREN IN SOME AREAS OF QINGDAO 2018

摘要 目的分析2018年青岛部分地区儿童呼吸道感染病原体流行病学特征。方法收集2018年1—12月就诊于青岛市部分医院的急性呼吸道感染儿童14231例,采用间接免疫荧光法检测血清8种病原体的IgM抗体,包括甲型流感病毒(IFA)、乙型流感病毒(IFB)、副流感病毒(PIV)、呼吸道合胞病毒(RSV)、腺病毒(ADV)、肺炎支原体(MP)、肺炎衣原体(CP)和嗜肺军团菌(LP),分别按性别、年龄、发病月份分组对结果进行统计学分析。结果14231例病儿共检出IgM抗体阳性8082例,阳性率为56.79%,其中男性阳性率52.97%,女性阳性率61.64%,男女比较差异有统计学意义(χ^(2)=107.590,P<0.001)。IgM抗体阳性率前3位的病原体是MP、IFB和IFA,阳性率分别为44.95%、29.53%和13.17%。IgM抗体阳性率年龄0~6月组最低(18.67%),6~9岁组最高(74.30%)。全年病原体IgM抗体总阳性率均在40.00%以上,8种病原体IgM抗体不同月份阳性率比较差异有统计学意义(χ^(2)=29.237~458.194,P<0.05),其中MP阳性率全年均较高,IFA夏季较高。结论2018年青岛部分地区儿童呼吸道病原体的感染率较高,以MP和流感病毒多见;混合感染常见。 Objective To analyze the epidemiological characteristics of pathogens causing respiratory infection in children in some areas of Qingdao,Shandong Province,China in 2018.Methods The clinical data of 14231 children with acute respiratory infection were collected from several hospitals in Qingdao from January to December 2018.Indirect immunofluorescence assay was performed to measure the serum immunoglobulin M(IgM)antibodies against eight pathogens,including influenza A virus(IFA),influenza B virus(IFB),para-influenza virus(PIV),respiratory syncytial virus(RSV),adenovirus(ADV),Mycoplasma pneumoniae(MP),Chlamydia pneumoniae(CP),and Legionella pneumophila(LP).The cases were grouped according to sex,age,and month of onset;the results were statistically analyzed in groups.Results Among the 14231 pediatric patients,8082(56.79%)showed positive results for IgM antibodies.There was a significantly difference in positive rate between boys(52.97%)and girls(61.64%)(χ^(2)=107.590,P<0.001).The top three pathogens according to the IgM antibody positive rates were MP(44.95%),IFB(29.53%),and IFA(13.17%).The group aged 0-6 months had the lowest IgM antibody positive rate(18.67%),and the group aged 6-9 years had the highest(74.30%).The overall positive rate of IgM antibodies was above 40.00%all year round.There were significant differences in IgM antibody positive rates of eight pathogens between different months(χ^(2)=29.237-458.194,P<0.05).Specifically,MP had a relatively high positive rate all year round,and IFA had a relatively high positive rate in summer.Conclusion The children in some areas of Qingdao have a high infection rate with respiratory pathogens in 2018.MP and influenza viruses are frequently seen,and mixed infection is common.
